Instructions for Use A. Device Description The FINESSE® ULTRA Breast Biopsy System is composed of a driver and a probe and allows for obtaining and storing multiple samples with a single probe insertion. The components of the system are designed to operate safely when used together for diagnostic sampling of tissue during a biopsy procedure. The driver is a self-contained, handheld, reusable electro-mechanical vacuum-assisted biopsy device (Figure 1) that utilizes a rechargeable lithium-ion battery and comes complete with AC power accessories (Figure 2) and carrying case. The biopsy probe is composed of a hollow outer cutting cannula, an inner needle with a notched sample chamber, and an integrated coaxial cannula. The outer cannula and integrated coaxial cannula have sharpened cutting edges and the inner needle has a sharpened trocar tip. The driver utilizes DC operated motors to create a vacuum while simultaneously moving the outer cannula backward to expose the sample chamber to draw tissue in. The outer cannula is released and moves forward over the sample chamber to cut the tissue. The tissue sample is transported via the sample chamber to the sample container. The sample chamber is returned to the original position and is available to acquire another sample. The integrated coaxial cannula may be detached after the biopsy and remain in the breast to retain a track to the biopsy site when placing a tissue marker. Figure 1 ®

B. Indications for Use The FINESSE® ULTRA Breast Biopsy System is indicated to obtain tissue samples from the breast or axillary lymph nodes for diagnostic analysis of breast abnormalities. The instrument is intended to provide breast tissue for histologic examination with partial or complete removal of the imaged abnormality. The extent of histologic abnormality cannot be reliably determined from its mammographic appearance. Therefore, the extent of removal of the imaged evidence of an abnormality does not predict the extent of removal of a histologic abnormality e.g., malignancy. When the sampled abnormality is not histologically benign, it is essential that the tissue margins be examined for completeness of removal using standard surgical procedures. C. Contraindications 1. The FINESSE® ULTRA Breast Biopsy System is for diagnostic use only, NOT for therapeutic use. 2. The FINESSE® ULTRA Breast Biopsy System is contraindicated for those patients where, in the physician’s judgment, there is an increased risk of complications associated with percutaneous removal of tissue samples. D. Warnings 1. Patients who may have a bleeding disorder or who are receiving anticoagulant therapy may be at increased risk of complications. 2. As with any biopsy instrument, there is a potential for infection. 3. The driver should not be used in a MRI suite. 4. The driver charger should not be used in an operating room. 1

Reusing the probe bears the risk of cross-patient contamination as biopsy probes – particularly those with long and small lumina, joints, and/or FUHYLFHVEHWZHHQFRPSRQHQWV±DUHGLI¿FXOWRULPSRVVLEOHWRFOHDQRQFHERGÀXLGVRUWLVVXHV with potential pyrogenic or microbial contamination have had contact with the probe for an indeterminable period of time. The residue of biological material can promote the contamination of the probe with pyrogens or microorganisms which may lead to infectious complications. 9. Do not resterilize FINESSE® ULTRA Biopsy Probes. After resterilization, the sterility of the probe is not guaranteed because of an indeterminable degree of potential pyrogenic or microbial contamination which may lead to infectious complications. Cleaning, reprocessing and/ or resterilizing the probe increases the probability that it will malfunction due to potential DGYHUVHHIIHFWVRQFRPSRQHQWVWKDWDUHLQÀXHQFHGEWKHUPDODQGRUPHFKDQLFDOFKDQJHV $OOEUHDVWELRSVLHVVKRXOGEHSHUIRUPHGXQGHULPDJLQJJXLGDQFHWRFRQ¿UPWKHSUREH position relative to the target region to be sampled and to help mitigate the occurrence of a false negative biopsy. 11. The battery may only be replaced or disposed of by an authorized Bard Service and Repair facility. 12. Use only with supplied AC power accessories. Removing the AC adapter from wall power shall serve as isolation means. Do not use the FINESSE® ULTRA Biopsy Probe without the integrated coaxial cannula. The integrated coaxial cannula may be removed after the biopsy to retain a track to the biopsy site when placing a tissue marker. 2. The FINESSE® ULTRA Breast Biopsy System should only be used by a physician trained in its indicated use, limitations, and possible complications of percutaneous needle techniques. 3. Do not demonstrate sample acquisition sequence in air prior to performing a biopsy procedure. Damage may occur to the needle or cannula tip. 4. Do not attempt to remove the cover or modify the device in any way. F. Potential Complications 1. Potential complications are those associated with any percutaneous removal/biopsy technique for tissue collection. Potential complications are limited to the region surrounding the biopsy site and include hematoma, hemorrhage, infection, a non-healing wound, pain and tissue adherence to the biopsy probe while removing it from the breast. 2. As per routine biopsy procedures, it may be necessary to cut tissue adhering to the biopsy probe while removing it from the breast. G. Sample Button Loading the Probe into the Driver 1. Driver should be inspected for damage prior to use. If damage is observed, please contact Bard Service and Repair. Ensure that the driver has been adequately charged. The driver should be charged overnight after daily use. 2. Remove the probe from the package using aseptic technique by grasping the probe from below. Ensure that the probe remains sterile. 3. Insert the probe into the driver (Figure 4). 4. Remove the retaining pin (Figure 5). 5. Slide the probe carriage back ensuring the tab securely locks into place (Figure 6).

When pressed, the “PRIME/PIERCE” indicator light continuously illuminates orange indicating the probe is being retracted approximately 20-22mm inside the driver, the “SAMPLE” indicator light is off and the “SAMPLE” button is deactivated. Once the probe is retracted, the “PRIME/PIERCE” indicator light continuously illuminates green indicating the probe is primed, the “SAMPLE” indicator light is off and the “SAMPLE” button is deactivated. Pressing the “PRIME/PIERCE” button again advances the probe approximately 20-22mm into the breast. Using the appropriate imaging guidance, the probe’s SRVWSLHUFHSRVLWLRQFDQEHYHUL¿HG$IWHUWKHSLHUFLQJVHTXHQFHLVFRPSOHWHWKH“PRIME/PIERCE” and “SAMPLE” indicator lights continuously illuminate green indicating another piercing sequence may be performed or a sample may be acquired. 7KH“SAMPLE” button initiates the sampling sequence. When pressed, the “SAMPLE” indicator light continuously illuminates orange indicting the sampling sequence is in operation, the “PRIME/PIERCE” indicator light is turned off and the “PRIME/PIERCE” button is deactivated. During the sampling process, the vacuum is turned on, the probe’s cutting cannula is pulled back exposing the sample chamber, tissue is drawn into the sample chamber, the cutting cannula is released to cut the tissue, the tissue sample is transported to the sample container and the system resets for the next sampling or piercing sequence.

NOTE: The driver does not contain an on-off switch. The driver automatically recognizes the probe type, senses when the probe has been properly inserted and performs an initialization to synchronize the driver and probe. The driver sounds one beep and the “PRIME/PIERCE” and “SAMPLE” indicator lights display green indicating the probe is properly inserted. During the initialization the motor is audible. 6. After initialization is complete, remove the protective sleeve from the probe. NOTE: If the driver is left undisturbed for 120 seconds after loading a probe, the driver will enter a sleep mode to conserve battery charge. During sleep mode, the “PRIME/PIERCE” and “SAMPLE” indicator lights will turn off and the buttons will be deactivated. Once the driver is moved, a motion sensor is activated and the driver automatically turns on. The driver will not enter sleep mode if the probe is primed. Performing a Biopsy 1. Prepare the biopsy site using appropriate aseptic techniques and adequate local anesthesia. Appropriate precautions should be followed for any percutaneous procedure (personal protective gear such as gloves and glasses/shields, etc. should be worn). 2. Make a small incision or puncture the skin with a scalpel. 3. Using imaging guidance, insert the probe through the incision and position the tip to the appropriate depth: ,IWKHOHVLRQLVJRLQJWREHSLHUFHG3RVLWLRQWKHWLSRIWKHSUREHSUR[LPDOWRWKHHGJHRIWKH lesion. ,IWKHOHVLRQLVQRWJRLQJWREHSLHUFHG3RVLWLRQWKHVDPSOLQJFKDPEHUWRWKHFHQWHURIWKH target region. (The center of the sampling chamber is approximately 17mm from the tip of the probe.) 4. If piercing is desired, prior to inserting the probe into the breast press the “PRIME/PIERCE” button to retract the probe 20-22mm. After inserting the probe press the “PRIME/PIERCE” button to advance the probe 20-22mm into the region of interest prior to taking WKH¿UVWWLVVXHVDPSOH 127(,PDJHJXLGDQFHVKRXOGEHXVHGWRFRQ¿UPSUREHSRVLWLRQUHODWLYHWRWKHWDUJHWUHJLRQWR be sampled. NOTE: Do not leave the system unattended with the probe in the primed state. NOTE: During the initial positioning of the probe, it is recommended to advance the probe into the tissue using the optional piercing mode. During subsequent samplings, the physician may wish to forego this step. NOTE: The probe may be primed for piercing while inside the patient if additional piercing shots are desired to access dense tissue or lesions. 5. Begin tissue sampling by pressing the “SAMPLE” button. The sampling process proceeds automatically: A vacuum is created, the outer cannula of the probe is automatically retracted, tissue is drawn by vacuum into the sampling chamber and the outer cannula is released cutting the tissue. The tissue sample is then transported to the sample container. During this process the “SAMPLE” indicator light continuously illuminates orange and the motor will be audible. The sample container illuminates to aid in visualizing the tissue sample. After the tissue sample is deposited into the sample container the system resets to acquire another sample if desired. NOTE: Do not press on the sample container or remove the sample container while acquiring tissue samples. If the system detects an error related to the sample container it will illuminate WKH6$03/(LQGLFDWRUÀDVKLQJRUDQJHDQGVRXQGEHHSV5HPRYHWKHVDPSOHFRQWDLQHUIURP the probe, clear any tissue underneath the container or from the probe where the container is VHDWHGLQVHUWWKHFRQWDLQHUEDFNLQWRWKHSUREH7KH6$03/(OLJKWZLOOÀDVKJUHHQWRLQGLFDWH that the sample container is properly inserted. Press the SAMPLE button to resume. The system will show the SAMPLE light continuous green. If error is not resolved see Troubleshooting. 6. To acquire multiple samples, repeat Step 5 above. Remove the tissue samples from the sample container by twisting the lid counter clockwise. If additional samples are required, ensure the sample container lid is in place while acquiring samples. NOTE: The sample container may be removed completely from the probe by pulling up on the lid and may be removed before or after the probe is removed from the driver. If the probe is loaded into the driver and the sample container is removed from the probe, the “PRIME/PIERCE” and ³6$03/(´LQGLFDWRUOLJKWVZLOOÀDVKRUDQJH$IWHUWKHVDPSOHFRQWDLQHULVUHLQVHUWHGLQWRWKH probe, the “PRIME/PIERCE” and “SAMPLE” indicator lights will continuously illuminate green. 9. Remove the probe from the driver by pressing down on the locking tab, pushing the carriage completely forward and pulling the probe straight from the driver (Figure 8). Figure 8

NOTE: Once a probe has been removed from the driver following a procedure, the probe cannot be re-inserted. If additional tissue samples are required, insert a new probe into the driver. NOTE: Driver will initiate mechanical reset following probe removal. Do not insert a new probe prior to completion of this reset. NOTE: The probe, coaxial cannula and sample container are for single use only. After use, these products may be a potential biohazard. Handle and dispose of in accordance with accepted medical practice and applicable local, state and federal laws and regulations. I. Cleaning and Maintenance $IWHUHDFKXVHFOHDQWKHGULYHUWKRURXJKOZLWKDJHUPLFLGDOGLVSRVDEOHZLSHVXFKDV6DQL&ORWK® or a damp cloth moistened with a 10% bleach solution. Other than routine cleaning, no other preventive maintenance is required. However, it is recommended to return the device to an authorized Bard Service & Repair Facility once per year for inspection and servicing. 'RQRWVSUDWKHGULYHUZLWKDQÀXLGV6SUDLQJWKHGULYHUPDFDXVHLWWRPDOIXQFWLRQDQGZLOOYRLG the warranty. Do not autoclave. Do not heat over 54°C (129°F). The charging stand may be cleaned with same methods as the driver; however, avoid wiping the charging terminal area. Do not clean charging stand while plugged in. 5

J. Driver Charging 7KHGULYHUXWLOL]HVDUHFKDUJHDEOHOLWKLXPLRQEDWWHU&RPSOHWHOFKDUJHWKHGULYHUSULRUWR¿UVWXVH7KH EDWWHUPXVWEHDGHTXDWHOFKDUJHGSULRUWRXVLQJWKHGULYHUIRUWKH¿UVWWLPHDVZHOODVSULRUWREHJLQQLQJ each procedure. The battery charge level may be checked by pressing the “SAMPLE” button while the driver is not loaded with a biopsy probe and is not in the charging stand (Figure 2). If the battery is VXI¿FLHQWOFKDUJHGWRSHUIRUPDELRSVSURFHGXUHWKHEDWWHULQGLFDWRUOLJKWZLOOFRQWLQXRXVOLOOXPLQDWH JUHHQIRUVHFRQGV,IWKHEDWWHULVQRWVXI¿FLHQWOFKDUJHGWRSHUIRUPDELRSVSURFHGXUHWKHEDWWHU indicator light will continuously illuminate red for 5 seconds and the driver must be charged. If the battery is completely discharged, the battery light will not illuminate and the driver must be charged. To charge the driver, follow these instructions: 1. Ensure the appropriate adapter plug is connected to the AC power adapter. 2. Connect the AC adapter cord to the charging stand. 3. Plug the AC power adapter into a power source. Once the battery is fully charged, the battery indicator light will continuously illuminate green. A completely exhausted battery will require approximately 120 minutes to fully charge. After every use or when the driver is not in use, the driver should remain in the charging stand to keep the battery adequately charged. A regulator restricts overloading the battery. NOTE: Use only with supplied AC power accessories. Removing the AC adapter from wall power shall serve as isolation means.
