INTENDED USE OF ELECTRIC SUCTION UNIT VICTORIA The electric suction unit VICTORIA, product line11-11xx, produced by CHEIRÓN a.s. Company follows up previous successful line of electric suction units. The electric suction unit Victoria is intended for use in medical care, primary for clinical applications such as secreta aspiration, suction blood from operating field, etc. Universal accessories allow its wide range using in hospitals, private surgery and sanatoria.

SAFETY INFORMATION
5.1
Instructions before Putting the Device into Operation Using application of the electric suction unit Victoria is intended by the producer only for medical purposes. The electric suction unit Victoria can be operated only by qualified personnel. Users are obligated to become familiar with instructions contained in this manual prior to using the device and follow them. The electric suction unit Victoria is intended for permanent duty. Device maintenance could be provided in regular intervals prescribed by the producer, i.e. CHEIRÓN a.s. Company. Only trained professional technicians (in accordance with regulations issued by CHEIRÓN a.s. Company) may perform repairs and maintenance. We recommend sign with CHEIRÓN a.s. Company or another authorized organization Service Contract about repairs and regular maintenance. The producer recommends to perform regular preventive maintenance of the device every 12 months, including written protocol. User is obliged to arrange for appropriate operation of the device: • possibility of in-place repair • device shipment to service • device shipment to producer Do not use the device, when it is damaged or unable to be set. Immediately arrange service inspection or repair. Do not use the device, until you ensure that it is completed and all its functions properly operate. Fuse replacement should be carried out only by properly qualified person.
5.2
Warnings The device is not intended for explosion hazard environment. Do not use any flammable matter for cleaning. Do not immerse the unit into water, nor wash it under running water. Prior to any cleaning or another manipulation not related to clinical operation, disconnect the unit from electric power supply. To dismount the suction jar holder (PORTABLE model) or another accessory is not allowed. Only trained professional technicians, in accordance with regulations issued by the producer, can carry it out. The producer prescribes using of MSF bacteria suction filters to stop any infection dissemination from suction jars. The MSF filter is joined to the connection tube (i.e. the tube between the suction jar and vacuum supply). The MSF filter should be regularly replaced (in 24 hours at least). Usage of MONOKIT OR FLOVAC disposable secreta bags is the easiest, safety and complex solution, because they already include microbiological filter. Handle suction jars carefully to protect the ambient environment from contamination.

To ensure patient’s safety, use the limiting water valve and any other accessories for the electric suction unit Victoria exactly in accordance with the Operator’s Manual. If the limiting water valve is used, opening of the inner tube on the top part of the limiting water valve (intended for vacuum level setting) must be all the operation time open – free-flow. This tube opening must never be closed.
Closing that tube opening could cause serious damage of patient. For unit installation on the mobile stand (models VERSA, THORAX, LIPOS) use only genuine Cheirón parts.
5.3
Precautions Do not use suction jars, which are in whatever way damaged or are not for that using purpose intended by the producer – possible implosion hazard. Check the correct suction circuit assembling and suction jars covers position, to avoid vacuum level decreasing. When any liquid or particle is sucked in the unit, immediately contact authorized service. Quickly performed service action can prevent severe demage of the device. In case of staining with any biological matter, especially with blood, disinfect the device with virucidal preparation. Do not add any disinfectant (such as persteril, calcium hypochlorite, etc.) into suction jars to decontaminate infectious secreta during the suction job. Disinfectants affect plastics and could them damage. There is a risk of suction jars cracking and damage of any other unit’s part or accessories as well! Do not immerse suction jars into disinfectants. Do not wash the suction tubing by aspirating disinfectants into the suction jars. Flowing aggressive vapors could damage the vacuum pump or other unit components. Do not use any detergent for cleaning plastic components. Do not use any solvent, abrasive cleanser or disinfectant for suction jars cleaning. Using of such like these cleaning preparations could cause irreversible damage of the suction jars. Do not use any chemical disinfectant for sterilization plastic components. Sterilize them only in autoclave! All autoclavable components prior to being placed into autoclave should be properly dried. The vacuum pump is equipped with a thermal fuse. It shuts down the unit when the device is overheated. In this case, switch off the unit using the main switch and wait at least 30 minutes. Before next use the unit, check the suction and connection tubing not to be clogged. The vacuum pump can be overheated due to the higher power supply voltage. In that case, contact Service Department of CHEIRÓN a.s. Company. For thoracic drainage use only screw cover suction jars. Non-sterile accessories intended for use with the suction device should be cleaned prior to use, according to instructions referred in this manual or according to your local internal regulations.

Notes The electric suction unit Victoria is class II electrical equipment with type B applied part, in accordance with standard ČSN EN 60601-1:1994. It is connected to power supply main via non-detachable two-wire power cord. The device is produced such a way to meet all the current technical and legislative standards. When the unit is on, but does not suck, check correct assembly of the suction system as well as the individual sections of the suction circuit for leak. Turn the vacuum regulator in (+) direction onto maximum vacuum level. The vacuum level set by the vacuum regulator is displayed on the vacuum gauge, only if the suction system is closed, e.g. if the connecting or suction tube is cranked or pressed between fingers. Specific maximum vacuum level -90 kPa (-93 kPa) is related to the sea level. According to your local conditions, i.e. according to the barometric pressure, small deviations can occur. See below Table 1 containing a couple of factors, which serve as reference values. Maximum vacuum level has to be multiplied by that factor, which corresponds to your actual local conditions. Prior to use the suction jar with the overfill protection, always check, if the float is in the float cage (on the internal side of the suction jar cover), with the sealing ring faced to the cover. There is no need to join the MSF suction filter to the suction circuit when using the MONOKIT or FLOVAC secreta bag. By use of the MONOKIT or FLOVAC secreta bags it is not necessary to sterilize the suction jar. All information concerning to accessories and spare parts you can get from the producer, CHEIRÓN a.s. Company or at www.cheiron.cz. During thoracic drainage, the suction jars as well as the limiting water valve should be always located lower from patient. The safety check valve in the limiting water valve makes to the suction flow a resistance, related to tube’s submerge length (in cmH2O). Thus the vacuum level, set on the limiting water valve outer tube, is lowered. Therefore, you should eliminate this vacuum loss this way, that you will submerge the inner tube for vacuum level setting deeper, to get your desired vacuum level. When two suction jars are used together and are interconnected, remove the overfill protection from the cover on the second suction jar (i.e. that jar, on which the suction tube is connected – the patient suction circuit).

BASIC INFORMATION The electric suction unit VICTORIA is a modern compact device intended for effective suction jobs. It is useful especially for service in operating theatres, recovery rooms, ICUs, wards, and for homecare or private surgery. Oil-free vacuum pump ensures the high suction output and needs no maintenance. By means of the vacuum regulator one can regulate vacuum level in range of -7 kPa to -90 kPa (-93 kPa). Vacuum level is monitored on the vacuum gauge. The electric suction unit Victoria is delivered with 2 L polycarbonate or polysulphon suction jars, alternatively with 4 L polycarbonate suction jars. It is possible to use 2 L polycarbonate suction jar in combination with the MONOKIT or FLOVAC disposable secreta bags. All suction jars are equipped with the overflow protection. The producer prescribes using of the MSF bacteria suction filters to stop any infection dissemination from suction jars. The individual model variants of the electric suction unit Victoria have identical electric circuitry and design. They differ each from the other in delivered accessories, i.e. in their using purposes. The electric suction unit Victoria is on the front side equipped with information for users (in the form of symbols). On the unit’s rear side, there is the identification label with producer identification data, technical data and some more symbols for users. The electric suction unit Victoria does not include any software nor is controlled by any additional software. The device including all accessories is delivered as non-sterile.

TECHNICAL DESCRIPTION OF DEVICE The electric suction unit Victoria consists of the stand-alone electric suction unit and the accessories.
9.1
Stand-alone Electric Unit
9.1.1
Electric Part The device consists of the electric membrane oil-free vacuum pump equipped with the thermal fuse, cooling system and electric circuitry. The device is connected into power AC outlet via non-detachable power cord terminated with the two-pin plug.
9.1.2
Pneumatic Part The mechanical vacuum regulator is located on the control panel. It controls vacuum level, value of which is monitored on the built-in vacuum gauge. The detachable oversuction jar is a part of the unit.

9.2.1
Foot Switch The pneumatic foot switch serves for switching the device On/Off during the suction job. It can be used when the staff working post is far from the device – approx. up to 2 m. The device should be switched on by means of the main switch before using the foot switch. Note: After finishing the suction job, switch the device off by means of the main switch, while the vacuum pump is running. If the device is switched off by means of the foot switch, which is then disconnected, at next switching the device on by means of the main switch, the vacuum pump will not start to run, in spite of the main switch indicator lights in green colour. You should give the demand of installation this option into the system in your order of the suction unit. Its additional installation is only possible at the producer’s workshop. Fig. 2 – Foot Switch

9.2.2
Suction Jar Change-over Switch The suction jar change-over switch is used for dual jar suction job. By switching the change-over switch you can choose, which suction jar will be just used for the aspirated secreta. Comparing to the standard serial suction jar configuration without change-over switch (see Chapt.10), in the changeover switch configuration you can change or clean any of two suction jars individually.

9.2.3

9.2.5
FLOVAC Disposable Suction Jar FLOVAC disposable suction jars are equipped with the hydrophobic filter. After filling up the jar, close its valves and dispose them included secreta, according to your local internal regulations.

9.2.6
MONOKIT Disposable Secreta Bag Using the MONOKIT disposable secreta bags allows to dispose secreta without necessity to replace the suction jar. The MONOKIT bag is intended for using with 2 L suction jar and it is disposable. It consists from the bag itself, to which the non-detachable cover is attached. Inside the MONOKIT cover, there is the hydrophobic bacteria filter, which filters contaminated passing air and simultaneously serves as the overfill protection. It prevents secreta to overflow into the unit and thus contaminate the whole system. When the bag is full of secreta, close both inlet and outlet on the MONOKIT cover. This prevents that infectious secreta could eventually escape from the MONOKIT bag (see Fig. 7,8). Use of the FLOVAC disposable secreta bags is identical with the MONOKIT ones.

Note: By use of the MONOKIT or FLOVAC secreta bags, it is not necessary to sterilize the suction jar.
9.2.7
MSF Suction Filter Always use the MSF suction filter to join the connecting tube, located between the suction jar and the over-suction jar. The MSF suction filter filters aspirated air prior to it enters the unit. The MSF suction filter stops eventual dissemination of the contaminated air from the unit into ambient space as well as it protects the unit from oversuction. When using the MSF suction filter, please follow the user‘s instructions for MSF suction filter. The MSF suction filter’s service life is maximum 24 operating hours. After use, dispose it according to your local hygienic regulations. Do not clean the filter. 9.2.8
Suction Tube 1.5m, PVC The suction tube is delivered at 1.5 m length and together with the suction adapter with finger suction interruptor creates the patient part of the suction circuit, i.e. that part between the patient and the suction jar. The PVC suction tube is disposable and is delivered as a standard accessory. Dimensions: Internal diameter: 8 mm Outer diameter: 12 mm The PVC tube can be also ordered as footage, with the finger suction interruptor not-included (see Chapt. 14). Fig. 10 – PVC Suction Tube Identical tube is commonly used for the inner suction circuit, i.e. for connection between the unit and the suction jar.
9.2.9
Suction Tube 1.5m, silicone The suction tube is delivered at 1.5 m length and together with the suction adapter with finger suction interruptor creates the patient part of the suction circuit, i.e. that part between the patient and the suction jar. The silicone suction tube is reusable. The silicone suction tube is option, delivered on customer’s demand. It substitutes the PVC suction tube. Dimensions: Internal diameter: 8 mm Outer diameter: 14 mm The silicone tube can be also ordered as footage, with finger suction interruptor not-included (see Chapt. 14). Fig. 11 – Silicone Tube
9.2.10 Suction Adapter with Finger Suction Interruptor This is the suction adapter connected in the proximal end of the suction tube (i.e. patient circuit). In the suction interruptor, there is the orifice for suction regulation: closing the orifice by finger, you start the suction, relief the orifice stops the suction. The suction interruptor can be substituted by the stop valve (see paragraph 9.2.10), using of which maintains stable vacuum level in the suction circuit, even if the suction procedure is interrupted. 9.2.11 Stop Valve, Disposable The stop valve is intended to easy control the suction job. Hospital staff is not in touch with aspirated secreta during the suction job.

Connect the stop valve rear side (1) to the proximal end of the suction tube (instead of the suction interruptor). Connect the catether onto the stop valve nozzle (2). If the orifice (3) is not closed by finger, there is no suction effect. On the contrary, when a finger closes the orifice (3), the dynamic suction process will start by means of vacuum created in whole system before. Another use of the stop valve: This item is useful as the supplement for suction process with aspirating into the suction jars connected to the hospital central suction system. The whole system can be actively connected to the hospital central wall vacuum, and thus be controlled only by the stop valve.
9.2.12 Container for Putting Catethers Aside This polycarbonate 40 cm long cylinder is designed for putting suction catethers aside. The container is autoclavable and detachable upon the eurorail by means of the holder. You can choose the twin holder as well.

9.2.13 Limiting Water Valve, Double, incl. Safety Check Valve
PATENTED 9.2.13.1 Using of Limiting Water Valve The dual limiting water valve is used only on the Victoria Thorax model to ensure safety thoracic suction job. Its purpose is to prevent the suction circuit from uncontrolled vacuum level rising, which could result in the lung injury (pneumothorax), and thus cause serious damage of patient. Safety vacuum levels are in range from approx. 10 cmH2O up to 40 cmH2O. The limiting water valve is equipped with the safety check valve, which prevents the thorax cavity from ambient air entry in case of vacuum supply failure. Thanks to its unique patented design, the Victoria Thorax enables to aspirate with full suction output, doing thoracic drainage simultaneously.

WARNING! To ensure patient’s safety, use the limiting water valve exactly in accordance with the Operator‘s Manual.
WARNING! If the limiting water valve is used, the opening of the inner tube on the top part of the limiting water valve (intended for vacuum level setting) must be all the operation time open – free-flow. This tube opening must never be closed.
Closing that tube opening could cause serious damage of patient. Note: Connect the limiting water valve into the suction circuit between the suction jar and the oversuction jar. Note: During thoracic drainage, the suction jar as well as the limiting water valve should be always located lower from patient.

Put water into the limiting water valve outer tube (3), up to approx. 2/3 of its maximum capacity (according to required vacuum level), in following way: Withdraw the inner tube for vacuum level setting (5) and lock it by means of the locking nut (6). Then unscrew the outer tube (3) counterclockwise, fill it with water, screw it carefully back and reasonably retighten (attention for thread damage). Unlock the locking nut and slide the inner tube (5) back into the outer tube (3). Changing the submerged length, you can set the desired vacuum level in cmH2O (see the formula in paragraph 9.2.13.3). Lock the inner tube in its final position by locking the nut to avoid the leakage or spontaneous inner tube sliding. Put into the safety check valve tube (2) so much water, that after screwing back (the same procedure as in previous case) the submerged length of the inner tube is 35 cm. Fig. 17 – Dual Limiting Water Valve Note: The safety check valve (2) in the limiting water valve makes to the suction flow a resistance, related to tube‘s submerge length (in cmH2O). Thus the vacuum level, set by the inner tube (5) in the limiting water valve outer tube (3), is lowered. Therefore, you should eliminate this vacuum loss this way, that you will submerge the inner tube (5) for vacuum level setting more deep, to get your desired vacuum level at the patient side.
9.2.13.3 Vacuum Level Setting on the Limiting Water Valve To set the desired vacuum level, submerge the inner tube (5) under water level inside the outer tube (3). You can set your desired vacuum level by submerged lenght (see the formula below). Lock the inner tube (3) in its final position by locking the nut (6), to avoid the leakage or spontaneous inner tube (3) sliding. Formula for resultant vacuum level calculation:

P – resultant vacuum level (in cmH2O ) X – submerged length of the inner tube (3) for vacuum level setting ( in cm) Y – submerged length of the inner tube in the limiting water valve ( in cm) Example: Desired vacuum level at the patient side is 20 cmH2O. If tube‘s submerged length in the safety check valve (2) is 5 cm under the water level, so this value reduces the system output. For that reason submerge the inner tube (5) to the length of 25 cmH2O.

9.2.13.4 Limiting Water Valve Leak Test Connect one end of the connection tubing onto the over-suction jar connector and the other end to the vacuum supply outlet connector (4) on the limiting water valve. Switch on the unit and set the vacuum regulator to MAX. Close the through-flow valve (7) on the limiting water valve, clockwise. Block the proximal end of the suction tube (patient circuit) by finger. Now slowly turn the through-flow valve (7) counter-clockwise, and if there is no leak in the whole suction system, the limiting water valve will start "bubbling“ – i.e. suck-in the ambient air via the inner tube (5). This way, the limiting water valve is tested and ready for use. Vacuum level in the suction circuit cannot rise over the vacuum level calculated according to the formula for resultant vacuum level calculation (see paragraph 9.2.13.3). By turning the through-flow valve (7) reduce "bubbling“ intensity, so that the bubbling speed is approx. 3 bubbles per second. When the entire above test is executed, you can connect the patient.
9.2.14 Store Basket II Store Basket II is easily accessible from the rear side of the mobile stand. It serves for putting the noncontaminated disposables aside. Store Basket II can be additionally installed on the mobile stand without any additional adaptation.

9.2.15 2 L Suction Jar Holder for Eurorail This holder is intended for fixing the 2 L suction jar. Attach the holder on the eurorail (e.g. on the mobile stand) and put the suction jar into the holder.

9.2.22 Mobile Stand The electric suction units VERSA, THORAX and LIPOS version are prepared for installation on the mobile stand, which improves device mobility as well as makes it´s handling easier. You can order the mobile stand anytime later and attach the unit on it in easy way. All the models are ready for additional installation on the mobile stand. The integrated catether container can be attached on the rear side of the mobile stand – on demand (see Fig. 25). In this case, it is not possible to attach the store basket II on the device (see paragraph 9.2.14).
