All staff in the working area should know how to turn off the laser system in case of an emergency. The use of mobile phones is prohibited in the working area while using the device because it could interfere with its proper operation. Ensure that the laser system key is in a safe place when not in use. Electrical Connection Requirements The device must be connected to the electrical system in compliance with national electrical safety regulations. In accordance with European standards, the device is normally supplied with a cable with different plugs related to the different countries and models.

According to IEC/EN 60825-1:2007 all laser devices must be equipped with a remote block connector connected to the access doors of the room to block the emission of the laser when access doors are opened.

A suitable switch will be prepared by the client on access doors to the room where the device will be installed: when doors are closed the switch will become activated through a closed contact; in the case of multiple access doors, each must have its own switch, with contacts connected in series.

Gemäß IEC/EN-Norm 60825-1:2007 müssen alle Lasergeräte mit einem externen Sicherheitsschalter ausgestattet sein, mit dem die Laserstrahlung automatisch unterbrochen wird, wenn die Zugangstüren zum Raum, in dem der Laser verwendet wird, geöffnet werden. Hierfür wird vom Kunden an den Zugangstüren zum Raum, in dem das Gerät aufgestellt wird, ein geeigneter Schalter angebracht: Bei geschlossenen Türen gibt ein Kontakt die Bedienung des Lasergeräts frei. Bei Räumen mit mehreren Zugangstüren müssen alle Türen mit in Reihe geschalteten Kontakten ausgestattet werden.

Start-up Procedure

Before proceeding with the start-up procedure of the device, verify the correct connection of the following accessories: • Power supply cable • Interlock connector • Key switch • Foot pedal • Optical fiber

Also make sure the emergency red button is not pushed in.

S’assurer également que le bouton rouge d’arrêt d’urgence n’est pas enfoncé.

To turn on the device: • Put the main switch to the I position on the rear panel. • Turn the key switch in position (clockwise) to turn on the system.

Pour mettre le dispositif sous tension : • Amener le commutateur principal situé sur le panneau arrière en position I. • Tourner le commutateur à clé en position (en sens horaire) pour mettre l’appareil sous tension.

The control panel switches on and the wavelength of the laser device and other instructions are displayed. The device beeps for about 4 seconds and the white LED ring around the laser fiber connection port lights up.

El panel de control se enciende y se muestran la longitud de onda del dispositivo láser y otras instrucciones. El dispositivo emite señales acústicas durante unos 4 segundos y el anillo del LED blanco que se encuentra alrededor del puerto de conexión de fibra láser se enciende.

Il pannello di controllo si accende e viene visualizzata la lunghezza d'onda del sistema laser insieme con altre istruzioni. Il dispositivo emette un segnale acustico per circa 4 secondi e l'anello LED bianco attorno alla porta di connessione della fibra si accende.

Säkerställ också att den röda nödstoppsknappen inte är intryckt. För att sätta på enheten: • Sätt huvudbrytaren i I-läget på bakpanelen. • Vrid nyckelbrytaren till läget (medsols) för att sätta på systemet. General Laser Warnings, Precautions, and Complications The following general warnings, precautions, and complications should be considered when using the Swiss LaserClast. These warnings, precautions, and complications apply to all of the surgical specialties described in this chapter. For important information specific to a particular surgical specialty, such as Lithotripsy, read the corresponding section later in this chapter. Clinical applications must be performed by qualified surgeons trained in laser applications for endourology procedures.

General Laser Warnings Incorrect treatment settings can cause serious tissue damage; therefore, it is recommended that you use the lowest acceptable treatment settings until familiar with the instrument’s capabilities. Use extreme caution until the biological interaction between the laser energy and tissue is thoroughly understood. During endoscopic surgery, the possibility of complications and adverse events, such as hypothermia, fever, edema, hemorrhage, inflammation, tissue necrosis, or infection may occur following treatment. In extreme cases, death may occur due to procedural complications, concurrent illness, or laser application. Risk of explosion if flammable gas is used as general anesthetic. Oxygen levels in the direct surgical area must not exceed 50 percent. The risks of combustion, perforation, and laser-induced hemorrhage, any of which could cause death, must be fully explained to the patient. The flammability of methane gas must be considered when treating in or near the perianal area.

The manufacturer has no clinical information concerning the safety of laser treatment on pregnant women or breast feeding women. Refer to the appropriate delivery system instruction guide to follow instructions.

General Laser Complications The potential complications encountered in endoscopic laser surgery are the same as those normally encountered in conventional endoscopic surgery. Acute pain may occur immediately following laser therapy and may persist for as long as 48 hours. Immediately following laser therapy, the patient may experience fever and leukocytosis, which are commonly associated with tissue destruction. These generally resolve without treatment. Laser ablated tissue may become necrotic or infected after treatment. If a question of infection exists, appropriate treatment should be carried out. The following complications could be serious and could result in death: Patients may experience bleeding at the site of laser therapy. Post-treatment hematocrits are recommended to identify this potential complication. Sepsis can result from performing any surgical procedure. If a question of sepsis exists, appropriate evaluations should be made. Perforation or fistula may occur as a result of laser treatment. To diagnose perforations, patients must be carefully followed post-operatively with appropriate tests.

There is a risk of infection and scarring associated with any surgical procedure. Therefore, appropriate pre- and post-surgical care should always be practiced.

Complications and risks are the same of the conventional laser surgery. These include, but are not limited to, the following:

The laser should be used only on tissues that are fully visible. Do not use the laser if the desired target is not in view.

Complications and non-thermal risks: • Perforation, • Aspiration, • Induced hemorrhage, • Allergic reaction to medication, • Hypertension, • Arrhythmia, • Pain, • Distension due to gases, • Pneumothorax, • Infection.

Use of the laser on anatomical structures in proximity to known critical structures, such as large arteries, veins, bowel, ureter, bladder, etc., should be performed carefully to avoid inadvertent or unintended treatment of such structures. In pre-treatment studies, screen tumors that are in close proximity to known arteries or veins to locate those circulatory structures. There is an increased risk of back scatter (reflection) and forward scatter (penetration) when using the laser if the laser is in non-contact mode.

General Laser Precautions Take caution with patients who have had difficulty with previous endoscopic procedures. Ensure that the laser treatment is the most eligible treatment. Blood vessels up to 1 millimeter in diameter can be effectively coagulated with the Holmium YAG wavelength. In case the laser hemostasis cannot control heavy arterial and venous bleeding, electro cautery and/or suture (ligature) should be easily accessible. Use caution when treating patients who have recently undergone radiotherapy. Such patients may be at greater risk of tissue perforation or erosion. Discontinue laser treatment immediately if the patient develops any cardiopulmonary problems. 78

Clinical Parameters The Holmium YAG wavelength provides effective hemostasis without damaging surrounding or non-target tissues. Coagulation effectiveness can be affected by reducing the energy or power density incident on vascularized tissue in two ways. First, when the tip of the hand piece or fiber is in direct contact with the target tissue, the pulse energy and repetition rate settings on the control screen may be reduced. Second, the surgeon may elect to defocus the beam without changing the system controls by pulling the hand piece or fiber tip approximately 2 to 5 millimeters from the target tissue. 82

Urinary Lithotripsy Indications The following applications are indicated for urinary Lithotripsy while using the Holmium YAG wavelength: •

Endoscopic fragmentation of urinary (urethral, ureteral, bladder and renal) calculi.

Contraindications

Complications As with other endoscopic urologic procedures, there may be urine leakage following the laser procedure. The use of flexible endoscopes carries an equivalent incidence of stricture formation; these rates may improve with further advances in ureteroscope design. Although rare, loss of a kidney may occur as a result of the procedure or because of the stone itself. Read General Laser Complications for a list of additional complications.

The Holmium YAG wavelength must not be used in patients with the following conditions:

Inability to undergo endoscopic treatment.

Intolerance to anesthesia.

Pre-clinical and clinical testing have demonstrated that urinary calculi can be safely and effectively fragmented at power settings of 0.5 or 0.6 Joules and at a frequency of 5 or 6 Hertz.

Warnings Unexpected tissue damage may occur due to excessive power application. Refer to the Urinary Lithotripsy Clinical Parameters chapter for recommended initial power settings. Use of excessive power may result in inadvertent perforation of the ureter or damage to other urologic structures.

For effective fragmentation, the tip of the optical fiber should be directly in contact with the stone. Whenever possible, laser energy should be directed at the side of, or at weak points in the stone. The stone should be progressively reduced in size by slowly removing small fragments. Continuous irrigation should be used to wash away stone fragments and to provide cooling of the treatment site. Set Short Pulses Mode for Increased fragmentation

Read the General Laser Warnings chapter for a list of additional warnings. Precautions The laser should be used with an optical fiber delivery system in direct view and in direct contact with the targeted ureteral stone. To minimize the potential for stone migration up the ureter, laser energy should be directed to the side of the stone if possible, rather than the leading face. Maintaining low energy levels and repetition rates will reduce the potential for possible stone migration. Be aware of edematous folds of epithelium that may lie between the optical fiber and the stone; however, research suggests such folds are very rare. Basketing may be used with larger stone fragments that are relatively hard or tend to escape in a retrograde fashion up the ureter. Baskets, guide wires, and other ureteroscopic accessories may be damaged by direct contact with the laser treatment beam. The use of irrigation is recommended throughout the lithotripsy procedure to absorb any heat produced, to carry stone fragments out of the urinary system, and to enhance direct visualization. The rate of irrigation should be carefully adjusted to avoid flux of calculi into the kidney. Follow the instructions for use of the endoscopes and accessories concerning their combination with a laser delivery device. Special care must be taken when working with flexible endoscopes to avoid damage of the endoscopes by laser energy of the fiber (e.g. accidental firing inside the working channel of an endoscope or too close to the lens, work channel perforation by moving the laser fiber tip in a flexible endoscope). Per proteggere il personale del centro di assistenza e per motivi di sicurezza durante il trasporto e la spedizione, tutti i prodotti e gli accessori restituiti alla fabbrica per la riparazione o la revisione devono essere puliti, disinfettati e sterilizzati in conformità con il manuale di istruzioni. La riparazione può essere rifiutata se il prodotto o gli accessori pervengono in condizioni di contaminazione.
