Introduction The BariMaxxTM II Therapy System is designed for bariatric patients with maximum weight of 1000 lb (455 kg). The product provides flexible patient positioning capabilities including Trendelenburg and Reverse Trendelenburg, a pressure reduction mattress, and integrated scales. Width of the patient surface can be expanded while the patient remains on the bed. See the Accessory section of this guide for additional information on accessories for the BariMaxxTM II Therapy System. • MaxxAir ETSTM Pressure Relief Therapy System • AtmosAirTM Fit with SATTM Mattress Replacement System (MRS) • Trapeze System • Power Drive System
Indications • Large patients weighing up to 1000 lb (455 kg) • Large patients at risk for pressure ulcers • Large patients requiring daily weight monitoring
Contraindications The BariMaxx™ II Therapy System is contraindicated for: • Patients with a total weight in excess of 1000 lb (455 kg) including accessories • Unstable Vertebral Fracture • Cervical and Skeletal Traction
Maximum Recommended Patient Weight Total patient weight should not exceed 1000 lb (455 kg), including accessories. The use of accessories on the bed may decrease the patient weight capacity of the bed. Contact KCI for questions concerning the use of accessories.
Risks and Precautions Transfer - Precaution should be taken during patient transfer, including the locking of caster brakes and caster steering. Refer to Patient Transfer in the Patient Placement section of this guide. Side Rails and Restraints – WARNING: Use of restraints, including side rails, can be critical to patient safety. Serious injury or death can result from the use (potential entrapment) or non-use (potential patient falls) of side rails or other restraints. See related Safety Information. Patient Migration - Specialty surfaces have different shear and support characteristics than conventional surfaces and may increase the risk of patient movement, sinking and / or migration into hazardous positions of entrapment and / or inadvertent bed exit. Monitor patients frequently to guard against patient entrapment.

Skeletal Traction or Unstable Fracture (if not contraindicated) - With skeletal traction, unstable pelvic fracture, or any other unstable fracture (to the extent not contraindicated), maintain physician directed angle of articulation and guard against risks of patient migration or inadvertent deflation of surface when using MaxxAir ETSTM Pressure Relief Therapy System. Electromagnetic Interference – Although this equipment conforms with the intent of the directive 89 / 336 / EEC in relation to electromagnetic compatibility, all electrical equipment may produce interference. If interference is suspected, move equipment away from sensitive devices or contact the manufacturer. Shock Hazard – Electrical shock hazard; do not open any electrical covers on bed, power drive or MaxxAir ETSTM Therapy Unit. Refer to qualified service personnel.
Safety Information Ambulatory Patient Entrance / Exit - Caregiver should always aid patient in entering and exiting the bed. Lower patient surface completely during assisted patient entrance / exit. Avoid Fire Hazards - To minimize the risk of fire, connect all power cords directly into a wallmounted outlet. Do not use extension cords or multiple outlet strips. Avoid Strains - Extra care should be taken when moving patients to and from the bed to avoid caregiver muscle strains. Make sure any caregivers who will be assisting the patient to and from the bed are physically capable of doing so. Always use a transfer device, when available. Bed Height – To minimize the risks of falls or injury the bed should always be in the lowest practical position when the patient is unattended. Make sure area under and around bed frame is clear of objects, persons, and parts of body before adjusting height. Bed Expansions - Extensions and side rails should always be locked in place whenever expanded or retracted. Blank Display - If main control panel remains blank, call for service immediately. Brakes - Lock all steer casters in line and set all caster brakes before transferring patient. Disposal - At the end of useful life, dispose of waste according to local requirements or contact the manufacturer for advice. There may be special requirements for disposal of batteries and leaded foam (if present in this product). Fluids- Avoid spilling fluids on bed controls. If spills do occur, unplug the bed, clean fluid from controls wearing rubber gloves to avoid any possibility of shock. Once fluid is removed, check operation of components in area of spill. Fluids remaining on controls can cause corrosion, which may cause components to fail or operate erratically, possibly producing potential hazards for patient and staff. General Protocols - Follow all applicable safety rules and institution protocols concerning patient and caregiver safety. Hand Control Lock - The lock-out function should be used at caregiver’s discretion to ensure against unintentional or unauthorized operation of bed functions. Moving Parts - Keep all equipment, tubes and lines, loose clothing, hair and parts of the body away from moving parts and pinch points. Oxygen Use - Ensure that the hand control is not contained in an oxygen enriched environment. Possible fire hazard when bed is used with oxygen administering equipment other than the 8
nasal prongs, mask, or half bed length tent type. Oxygen tent should not extend below mattress support level. Power Cord - Ensure power cord is kept free from all pinch points and moving parts and is not trapped under casters. Improper handling of the power cord can cause damage to the cord, which may produce risk of fire or electric shock. Scale Readings - Scales / patient weights are for reference only. Scale readings should not be relied upon for medication dosage. Side Rails / Patient Restraints – Whether and how to use side rails or restraints is a decision that should be based on each patient’s needs and should be made by the patient and the patient’s family, physician and caregivers, with facility protocols in mind. Caregivers should assess risks and benefits of side rail / restraint use (including entrapment and patient falls from bed) in conjunction with individual patient needs, and should discuss use or non-use with patient and / or family. Consider not only the clinical and other needs of the patient but also the risks of fatal or serious injury from falling out of bed and from patient entrapment in or around the side rails, restraints or other accessories. For a description of entrapment hazards, vulnerable patient profile and guidance to further reduce entrapment risks, refer to FDA’s Hospital Bed System Dimensional and Assessment Guidance To Reduce Entrapment, and other information referenced at www.kci1.com/products/FDASafetyAlert or www.fda.gov. Consult a caregiver and carefully consider the use of bolsters, positioning aids or floor pads, especially with confused, restless or agitated patients. It is recommended that side rails (if used) be locked in the full upright position when the patient is unattended. Make sure a capable patient knows how to get out of bed safely (and, if necessary, how to release the side rails) in case of fire or other emergency. Monitor patients frequently to guard against patient entrapment. It is recommended that electrically operated beds conform to IEC 60601-2-38. Medical Electrical Equipment Part 2: Particular requirements for the safety of electrically operated hospital beds. To help prevent inadvertent bed exit or falls, ensure the distance between top of side rails (if used) and top of mattress (without compression) is approximately 4.5 inches (11.4 cm) or greater. Consider individual patient size, position (relative to the top of the side rail), and patient condition in assessing fall risk. Skin Care - Monitor skin conditions regularly, especially at bony prominences and in areas where incontinence and drainage occur or collect, and consider adjunct or alternative therapies for high acuity patients. It is recommended that skin pressure points be assessed at least every two hours. Early intervention may be essential to preventing serious skin breakdown. Turning - When using a MaxxAir ETSTM Pressure Relief Therapy System, prior to engaging turn feature, ensure that bed frame has siderails and that all siderails are fully engaged in their full upright and locked position. Transfer Board - If a mechanical lift device is not available, a transfer board should always be used when transferring a patient to and from the BariMaxxTM II Therapy System. Headboard / Footboard Transport Bar -The headboard / footboard transport bar is intended to assist with bed transport. It is not recommended for use by patient in repositioning or transfer. BariMaxxTM II Therapy System equipped with a Power Drive System - to minimize the risk of serious injury, carefully read and follow all Safety Information and Operating Instructions before using the Power Drive System (refer to BariMaxxTM II Power Drive User Manual P/N 310956 and Quick Reference Guide P/N 310971). Ensure all physician orders and facility protocols are followed. 9
Patient Placement It is recommended that all sections of this guide be reviewed prior to product use. Carefully read the Risks and Precautions and Safety Information in the Introduction section prior to placing a patient on the BariMaxxTM II Therapy System. Power Drive System - For BariMaxxTM II Therapy Systems equipped with a BariMaxxTM II Power Drive System refer to the BariMaxxTM II Power Drive System User Manual (P/N 310956) for Placement Instructions, Risks and Precautions and Safety Information. MaxxAir ETSTM Pressure Relief Therapy System - For BariMaxxTM II Therapy System equipped with a MaxxAir ETSTM Pressure Relief Therapy System refer to the MaxxAir ETSTM User Guide (P/N 310115) for Placement Instructions, Risks and Precautions and Safety Information.
Preparation for Patient Placement 1.
Verify bed is positioned in the specific room location requested by caregiver.
2.
Lock caster wheels and caster brakes in place.
3.
Plug power cord into a properly grounded 115V AC, 15 Amp wall outlet. Do not use a wall outlet that is controlled by a wall switch.
4.
Press green on / off rocker switch on the motor module switch located at the foot-end of the bed to the on (lighted) position.
5.
Adjust bed to the appropriate position for placement.
Patient Transfer 1.
Lock all steering casters and lock all caster brakes before transferring patient.
2.
Adjust bed height to optimize caregiver / patient safety during patient transfer, slightly lower than the bed the patient is exiting.
3.
Lower all side rails.
4.
Place a transfer board between beds if necessary.
5.
Transfer patient to bed following all applicable safety rules and institutional protocols that may apply.
6.
Center patient from side-to-side on mattress.
7.
Ensure bed linens under patient are not wrinkled.
8.
Raise and lock all side rails. If using side rails read the Risks and Precautions and Safety Information in the Introduction section of this guide.
Completion of Patient Placement

1.
Adjust head and knee sections of bed for patient comfort.
2.
If the head section is raised, the knee section should also be raised (if not medically contraindicated) to prevent patient migration toward the foot-end of the bed.
Nursing Care It is recommended that all sections of this guide be reviewed prior to product use. Carefully read the Risks and Precautions and Safety Information in the Introduction section prior to performing nursing care for a patient on the BariMaxxTM II Therapy System. CPR 1.
Adjust bed to appropriate level.
2.
Lower side rails.
3.
Begin CPR. Use of backboard is recommended.
4.
After CPR is performed and patient is clinically stable: • Adjust head and foot elevation for patient comfort. • If using side rails read the Risks and Precautions and Safety Information in the Introduction section of this guide.
Caregiver Convenience The following BariMaxxTM II Therapy System features facilitate patient care: • A nurse display located on the foot board for bed positioning and adjustments • A hand control for convenient bed position adjustment • Accessibility for patient lifts • A manual crank, stored below footboard, for bed position adjustments during power outage • Hand control lock-out feature which can be used to prevent unintentional or unauthorized operation of bed functions • Width expandability while the patient remains on the bed • Integrated scales for accurate patient weighing
Skin Care 1.
Follow facility guidelines for skin care.
2.
Remove excess moisture and keep skin dry and clean.
3.
Monitor skin conditions regularly, especially at bony prominences and areas where moisture or incontinence may occur or collect, and consider adjunct or alternative therapies for high acuity patients. Early intervention may be essential to preventing serious skin breakdown.
4.
Ensure linens under patient are not wrinkled.

Incontinence / Drainage 1.
Use moisture impermeable under-pads for incontinent patients.
2.
Watch for incontinence or drainage and provide appropriate skin care following each episode.
3.
Wipe mattress clean (follow facility protocols) and replace bed linens as required.
Patient Assisted Repositioning Using a Bed Sheet 1.
Position head and foot sections of bed as appropriate.
2.
Lock all steer casters in line and set all caster brakes.
3.
Lower all side rails.
4.
Take position on each side of patient (at least two people will be needed, possibly more depending on weight being lifted). Ensure bed sheet will hold the weight being moved. Roll edges of bed sheet on each side of patient to get a firm grip. If needed adjust Trendelenburg or Reverse Trendelenburg to assist in patient positioning.
5.
Use sheet to position and assist patient in achieving proper position on bed. Follow all applicable safety rules and institution protocols that may apply.
6.
Ensure bed sheet or other linens under patient are not wrinkled.
7.
Raise and lock side rails. If using side rails, read the Risks and Precautions and Safety Information in the Introduction section of this guide.
Patient Bathing 1.
Position head and foot sections of bed as appropriate.
2.
Lock all steer casters in line and set all caster brakes.
3.
Lower side rails on caregiver's side only, or as needed.
4.
Bathe and rinse patient's anterior.
5.
Roll patient into side-lying position.
6.
Bathe and rinse patient's exposed side and posterior. Avoid spilling fluids on bed controls. If spills do occur, unplug the bed, clean fluid from controls wearing rubber gloves to avoid any possibility of shock. Once fluid is removed, check operation of components in area of spill. Fluids remaining on electronic controls can cause corrosion, which may cause the electronic components to fail. Component failures may cause the bed to operate erratically, possibly producing potential hazards to patient and caregiver.

7.
Push soiled bed linens to center of bed.
8.
Wipe and rinse any soiling on the mattress surface (follow facility protocols) while the patient is in side-lying position.
9.
Dry mattress with towel.
10.
Place clean linens over clean side of mattress and tuck in sides.
11.
Roll patient to clean side of mattress and repeat steps 6 through 10 for the opposite side.
12.
Return patient to supine position in the center of bed.
13.
Ensure bed linens under patient are not wrinkled.
14.
Ensure side rails are in the fully raised and locked position. If using side rails read the Risks and Precautions and Safety Information in the Introduction section of this guide.
15.
Adjust head and foot of bed for patient comfort.
Bedpan Placement 1.
Adjust bed to appropriate level for patient comfort.
2.
Lock all steer casters in line and set all caster brakes.
3.
Lower side rails on caregiver's side only or as needed.
4.
Position bedpan parallel to patient's buttocks.
5.
Roll patient toward opposite side rails.
6.
Push bedpan down into mattress and position bedpan underneath patient.
7.
Roll patient onto bedpan, keeping one hand on bedpan.
8.
Ensure side rails are in the fully raised and locked position.
9.
Adjust head and foot of bed for patient comfort.
Bedpan Removal 1.
Adjust bed to appropriate level for patient comfort.
2.
Lock all steer casters and caster brakes.
3.
Lower side rails on caregiver's side only or as needed.
4.
Roll patient to opposite side rails.
5.
Grasp bedpan firmly with both hands, push down into mattress and slide bedpan out from under patient.
6.
Clean patient’s posterior and any soiling on linens or mattress.
7.
Return patient to supine position on mattress.
8.
Ensure side rails are in the fully raised and locked position. If using side rails read the Risks and Precautions and Safety Information in the Introduction section of this guide.
9.
Adjust head and foot of bed for patient comfort. 13
Patient Transfer to the BariMaxxTM II Therapy System Refer to the Patient Placement section of this guide if the patient is being transferred to the BariMaxxTM II Therapy System for the first time. MaxxAir ETSTM Pressure Relief Therapy System - For BariMaxxTM II Therapy System equipped with a MaxxAir ETSTM Pressure Relief Therapy System refer to the MaxxAir ETSTM User Guide (P/N 310115) for Placement Instructions, Risks and Precautions and Safety Information. 1.
Lock all steering casters and lock all caster brakes before transferring patient.
2.
Adjust bed height to optimize caregiver / patient safety during patient transfer, slightly lower than the bed the patient is exiting.
3.
Lower all side rails.
4.
Place a transfer board between beds if necessary.
5.
Transfer patient to bed following all applicable safety rules and institutional protocols that may apply.
6.

Transfer patient from bed, following all applicable safety rules and any institutional protocols that may apply. Refer to the Power-Down Procedure section in the Operating Instructions section of this guide if the patient will not be returning to the bed.

Care and Cleaning The following are the KCI recommended daily and weekly cleaning and infection control procedures for the BariMaxx™ II Therapy System while in use. If this product is to be purchased, or utilized for long term rentals, it is recommended that KCI be contacted for recommended infection control procedures to be utilized by the facility. It is recommended that all sections of this guide be reviewed prior to product use. Carefully read the Risks and Precautions and Safety Information in the Introduction section prior to performing cleaning procedures on the BariMaxx™ II Therapy System.
General Instructions
SOLVENT ALCOHOL
• Do not clean with solvents or alcohol. • Do not launder (machine-wash) any AtmosAir™ Fit MRS surface or mattress components.
SOAP
• For regular cleaning, use a mild detergent with water on a non-abrasive cloth. • To disinfect, use only approved disinfectants diluted in accordance with manufacturer’s instructions. • When blood and / or body fluids are present, use the following throughout all cleaning procedures: • Disposable, powder-free latex or latex-free gloves. • Protective clothing, including disposable or reusable impervious apron or gown. • Protective eyewear and face shield, as necessary.
Daily Care and Cleaning Daily care and cleaning consists of wiping down the surface and side rail covers (as needed) during patient bathing. The patient does not need to be removed from the bed when performing daily cleaning. Refer to Patient Bathing section for proper procedure.
Weekly Care and Cleaning In addition to daily care and cleaning, the following items should be wiped down weekly: BariMaxx™ II bed frame, side rails and pads, hand control, headboard, footboard, nurse control panel. The following BariMaxxTM II Therapy System accessories, when in use, should follow the cleaning procedures below. Power Drive System - In addition to the preceding cleaning instructions refer to facility protocols and the BariMaxxTM II Power Drive System User Manual (P/N 310956). MaxxAir ETSTM Pressure Relief Therapy System - In addition to the preceding cleaning instructions refer to facility protocols and the MaxxAir ETSTM User Guide (P/N 310115). Trapeze System - In addition to the preceding cleaning instructions, follow facility protocols. Avoid spilling fluids on any part of the main control panel or hand control. Fluids remaining on electronic controls can cause corrosion, which may cause the electronic components to fail. Component failures may cause the bed to operate erratically, possibly producing potential hazards to patient and caregiver. 15
Operating Instructions This section contains instructions for setting and adjusting functions of the BariMaxxTM II Therapy System. It is recommended that all sections of this guide be reviewed prior to product use. Carefully read the Risks and Precautions and Safety Information in the Introduction section prior to operating the BariMaxxTM II Therapy System. Power Drive System - For BariMaxxTM II Therapy Systems equipped with a BariMaxxTM II Power Drive System refer to the BariMaxxTM II Power Drive System User Manual (P/N 310956) for Operating Instructions, Risks and Precautions and Safety Information. On / Off Switch The on / off switch is a green lighted rocker-type switch located at the foot-end of the bed. Use this switch to power the bed on or off as needed.

Nurse Control Panel The nurse control panel is located at the foot end of the bed, in the middle of the footboard. It is the caregiver’s primary point of interface for controlling bed functions. Use the nurse control panel to: • ZERO (0.0) - Zero scale • MODE (LB / KG) - Convert weight display from pounds to kilograms • HOLD - Hold current patient weight during addition or removal of linens, equipment, etc.

• WEIGHT - Display current patient weight • WT/CH - Display any variation in patient weight • ON - Bring nurse control panel out of Sleep Mode • HEAD UP / HEAD DOWN - Raise / lower head of bed • BED UP / BED DOWN - Raise / lower height of bed • KNEE UP / KNEE DOWN - Raise / lower knee section of bed • TREND / REVERSE TREND - Position bed into Trendelenburg / Reverse Trendelenburg
Hand Control The hand control shown at right is the patient's primary point of interface for controlling the bed functions. Use the hand control to: • Raise / lower head of bed • Raise / lower height of bed • Raise / lower knee section of bed • Position bed into Trendelenburg / Reverse Trendelenburg
Power-Up Procedure 1.
Plug power cord into a properly grounded wall outlet. Do not use a wall outlet that is controlled by a wall switch.
2.
Press green on / off rocker switch located at the foot-end of the bed to the on (lighted) position.
3.
Adjust bed to the appropriate position for placement.
Head Section Adjustment The head section can be adjusted to create an upper body support angle from 0° to approximately 60°. When the head section is raised, the knee section should also be raised to prevent patient migration toward the foot-end of the bed. Use the buttons on the hand control or nurse control panel to raise or lower head section. 17 - Operating Instructions

Bed Height Adjustment The bed height can be adjusted from approximately 17.5 to 26 in from the floor. Use the buttons on the hand control or nurse control panel to raise or lower bed height.
Knee Section Adjustment The knee section can be adjusted to prevent patient migration toward the foot-end of the bed when the head section is raised. The knee section can be adjusted from 0° to approximately 30°. Use the buttons on the hand control or nurse control panel to raise or lower knee section.
Trendelenburg / Reverse Trendelenburg Adjustment The bed can be adjusted for Trendelenburg and Reverse Trendelenburg up to approximately 9°. Use the buttons on the hand control or nurse control panel to adjust Trendelenburg and Reverse Trendelenburg.

Scale Operation The BariMaxxTM II Therapy System features integrated scales to provide accurate patient weighing. Use the nurse control panel to access all scale functions. For most accurate weight readings, the bed must be level (not in Trendelenburg or Reverse Trendelenburg). ZERO (0.0) - Press to zero scales prior to patient placement, with linens and any other equipment already on bed. 1.
Press and hold the 0.0 button. HOLD TO ZERO will appear in the nurse control panel display.
2.
When the message in the nurse control panel display changes to PLEASE WAIT - HANDS OFF, release the button and do not touch the bed. In a few seconds, the nurse control panel display will read WEIGHT = 0.0 LB.
MODE (LB / KG) - Press to convert weight display from pounds to kilograms. HOLD - Press to hold current patient weight during addition or removal of linens or equipment. The nurse control panel display will read HOLD = (patient weight). Press the HOLD button again to exit Hold Mode. While in the Hold Mode, the WEIGHT and WT / CH buttons can be used to manually adjust the patient’s weight in the event of a known error. WEIGHT - Press to display current patient weight and to bring the nurse control panel out of Sleep Mode. WT/CH - Use to monitor changes in the patient’s weight. 1.
Press the WT / CH button. The nurse control panel display will read WT / CH = (patient weight).
2.
Press the ZERO button to set the weight to zero. Any variation in weight will be displayed as a +XXX LB or -XXX LB.
3.
Press the WEIGHT button to return the scales to normal operation.

Emergency Crank Use the emergency crank stored below the footboard to adjust bed position if powered functions are not available. 1.
Pull emergency crank from mounting clips below footboard.
2.
Insert crank into desired drive shaft fitting shown below.
3.
Turn crank clockwise to raise a surface and counterclockwise to lower a surface.
4.
Store emergency crank in mounting clips.

Hand Control Lock Procedure 1.
Position bed as desired.
2.
Press the HAND CONTROL LOCK button on the nurse control panel to lockout the hand control.
3.
Press the HAND CONTROL LOCK button on the nurse control panel again to unlock the hand control.
Power-Down Procedure

Press lighted green rocker on / off switch to off position.
2.
Carefully unplug power cord from wall outlet.
Side Rail Operation The position of the side rail can be changed using the following procedure: 1.
Pull the release lever towards the top of the side rail.
2.
Swing side rail out as shown below to raise or lower.
3.
Let go of the release lever.
4.
Ensure side rail is securely latched. When using side rails refer to the Risks and Precautions and Safety Information in the Introduction section of this guide.

Bed Width Expansion The BariMaxxTM II Therapy System can be expanded in width using the following procedure: 1.
Rotate locking handle down.
2.
Pull side rail slide expansion out until locked in place.
3.
Add mattress inserts to the bed using buckles and zippers to secure in place. Ensure that all eight expansions are locked in place prior to installing mattress inserts.

Mattress Inserts After bed expansion, add the mattress inserts using the following procedure. Not applicable with the MaxxAir ETSTM Pressure Relief Therapy System. 1.
Place mattress insert in the gap between the side rails and the mattress with the zippers to the outside and the marked foot end at the foot end of the bed.
2.
Connect the four female buckles on the underside of the mattress inserts to the four male buckles on the underside of the mattress.
3.
Zip the flap on the mattress to the mattress inserts.
4.
Repeat steps 1-3 for the opposite side of the bed.

Caster Operation Steer-Lock Casters The BariMaxx™ II Therapy System is equipped with steer-lock casters at the foot end, which allow the bed to be steered from that end. The bed is usually easier to handle if only one end of the bed is set to steer. 1.
Push the steer-lock tab down with foot to lock steering.
2.
Raise steer-lock tab with foot to unlock steering.
3.
Caster should swivel freely when bed is moved.

Caster Brakes All four casters are equipped with brakes. These brakes prevent the bed from being moved once it is in position. 1.
To lock brakes, press the Red end of the brake lever down fully with foot so that the lever is not parallel with respect to the floor.
2.
Unlock the brakes by pressing the Green end of the brake lever down with your foot until the lever is parallel with respect to the floor.
