Correct Use
Elements of device and use
Basic equipment: THERA-vital basic equipment serves passive and active exercise as leg trainer with foot-rests. The adjustable grips will offer you especially while actively training safe hold. Option upper torso trainer: The upper torso trainer with therapy grips offers alternatively exercise fort the upper limp. It is like the leg trainer appropriate for passive training free of effort and active training.
THERA-vital is suited for use at home, in clinics, elderly homes or hospitals. It is meant for mobilisation of persons that are disabled in their movement after accidents, surgeries, or after general illnesses which concern the apparatus of movement and truss. THERA-vital can be used as a leg trainer and as a upper torso trainer. It is appropriate for active and passive exercise. It is not possible to make specific recommendations for the proper use of THERA-vital in regard to different diseases as well as details for a movement program, as those aspects are closely related to the patient’s individual state, his condition after surgery, age, height and the overall condition. Please see your doctor’s advice. THERA-vital is a therapeutic movement exerciser and no medical advice for diagnostic measures.

Safety ❑ Before using THERA-vital the first time or after a long transport please leave the device for around one hour at room temperature. ❑ Make sure THERA-vital is put on an even and anti-slip ground, so that the best possible stability is guaranteed. ❑ THERA-vital may only be used with the line voltage shown on the data plate. Please only plug the device into a proper grounded plug box. ❑ Set up the power cable in a way that nobody can stumble and that no damages can be caused by other devices. Do not use the THERA-vital, if you notice any defects in the power cable. Do always use original power cables. ❑ THERA-vital must not be used in wet, damp or very hot areas in order to prevent electric shock. ❑ Take out the power plug in any case before opening the casing. ❑ THERA-vital never can be used without the casing. ❑ Do not use any solvent-based fluency when cleaning the device. ❑ The extra use of an anti-slip device for the wheel-chair is necessary, if the wheel-chair might slip backwards for very active patients.

So start your training with a small distance. Avoid any overstretching of the joints. ❑ Immediately seek doctor’s advice, if any symptoms occur while you train or after the training. ❑ Please observe training children during the whole exercise. ❑ Playing children must always be in supervision.

THERA-vital is especially made for:
Planning the training
The goal of the therapy is closely related to the specific diseases of the patient and can be to obtain mobility on the one hand, on the other hand the rehabilitation after surgery or injury, which means getting back mobility and capacity for work. However, a training plan must be set by a therapist or a doctor. Start the training slowly and increase it until the individual capacity is reached. Please take care that the capacity of the patient will not be exceeded.

Usually the doctor or therapist plans or recommends how often and for how long the patient should exercise on an individual basis. Regular training with THERA-vital is extremely important for any improvement in mobility process. Short but regular use is better than long ones that are exhausting. So start with a maximum of 5 to 10 minutes training in a row – if your physical mobility allows you to do so. Choose a small resistance. You can increase the training time, speed and resistance in small steps, whereas the patient’s capacity level never must be overcharged. The therapy will be most successful, if there will be no exhausting state of any kind, the patient feels at ease during the whole time and strength and mobility slowly increase.
Application Range THERA-vital can be used as a leg trainer and as a upper torso trainer: ❑ Make sure the patient is seated on a chair or in a wheel-chair. THERA-vital is situated in front of the patient on the floor. For example the calf rests 4 or the foot fixing belts 14 are fixed at the foot rests 5. The console 2 activates the leg trainer. ❑ When using the upper torso/arm trainer the patient is situated in front of the device on a chair or in a wheel-chair. On the upper part of the trainer you fix the therapy grips 12 and maybe the arm rests 13. The console 2 activates the upper torso trainer.
Correct position of body Always make sure that the patient’s position is in accordance with the goal of the therapy while working with THERA-vital. Start with a small distance between device an patient to avoid an over-stretching of the joints or damage in the muscle-/tendon area. The bigger the distance between THERA-vital and the patient, the more the knee- and hip-joints or arm-shoulder-joints will be stretched. Using foot-rests 5 please take care that the feet are really fixed with the belts or with the foot-fixing belts 14 (see chapter “foot-rests”, page 31).

Active and Passive Training Passive training means the patient works without his own physical power, only by using THERA-vital’s motor power. After putting THERA-vital ON, it always starts with a passive training in pre-set power and number of revolutions. By giving active physical power into the same direction of rotation the patient can change into the active training. When training actively the patient is working against the adjustable resistance of the device by using his own physical strength. (Chapter “Treading resistance/Capacity settings”, page 35). The patient has several possibilities to train actively: Set of Resistance level: The patient works against a pre-set resistance (see page 35). Power demand mode: The patient works with constant capacity (Watt). Therefore the resistance will be adjusted in accordance to the number of rotations (see page 41). Speed demand (Isokinetic): The patient works with an adjusted number of rotations. The resistance will be adjusted in accordance to the muscle-power the patient puts on (see page 41).
If Spasm occurs Heart rate control mode: The patient works with an fix adapted heartrate. The resistance will be adjusted automatically in accordance to the measured heart rate.

Spasm can be relaxed easily with THERA-vital by doing a small and regular movement with a small pedal radius. It is important though that the distance between THERA-vital and the user of the device is being set as small as possible at first. If you make sure that your position is upright and the pedal radius is a small one (see “Shifting of the radius” page 31), spasm can be reduced after a short period of time in the most cases. According to experience and scientific research, this reduction of spasm can last up for several hours. The training program should be developped with a doctor or therapist in a way that helps to reach the right extent regarding any reduction of spasm. THERA-vital is being designed with "intelligent” electronics that recognizes spasm whenever it occurs and stops the engine. However, the Anti-Spasm-function must be turned on for this (operation see page 35). Most patients are showing the symptom of “stretching spasm”. Whenever stretching spasm occurs, THERA-vital stops and keeps on operating forward after a short break. By keeping to operate forwardly the muscle length is reduced and the spasm can relax (operation see page 39). Whenever “bending spasm” occurs, which is a rather unusual form of spasm, THERA-vital keeps on operating backwards, after spasm has set in (operation see page 39).
Transport and Putting into Operation Before first Use Take THERA-vital out and check it to see if there are any possible damages as a result of transport. Also check and see if the line voltage shown on the data plate as the same voltage shown on your power cable. In the case of any damage or wrong line voltage please immediately contact your deliverer.
Transport of the Device C THERA-vital is being designed with transport rolls as standard. In the case of transport simply incline THERA-vital backwards as shown in the picture and pull it right behind you. You have to carry the device if you are crossing any steps or stairways. Two people are needed for that.
Operation How to switch the device on and off
Foot-Rests
Is the display not lighted you can out THERAvital into action by pressing a button at will on the console 2. The display will get lighted. Is the display lighted you can start THERA-vital by pushing START on the console 2 and start with the training. You can stop your training by pushing STOP on the console 2 (pause). Pushing STOP once again will stop the training and the results will be shown on the display.
The foot-rests 5 are applicable for both the training with or without shoes. G You can easily fix your feet in the footrests 5 with the belts onhand. Therefore the belt is put over the foot and fixed on the outside of the foot-rest 5. Make sure before every training whether your feet are in the right position in the foot-rests 5 and are safely fixed with the belts.
D PC-Port
(Option) After the assembling of most suitable guide bars the handle length of the feet pedals can be set steplessly. This way an individual mobility level of the feet (Hub) can be reached for each patient individually. ❑ Switch THERA-vital off by pressing the ON/OFF button. ❑ Untie the locking star-shaped screw by hand. ❑ Bring the foot-rest in the desired position and tie the star-shaped screw! ❑ Set the pedal on the other side in the same way. Pay attention to get equal lever lengths (marks on the handles). The smaller the length of the handle, the higher the power on legs and joints.
The connection to the PC is directed to an RS232 interface 16. This interface makes it possible to update the software or the technical service.
Power Connection D Plug THERA-vital into the socket with the
power cable. Turn the line switch located in the back behind the bush on 15. The line switch lights green. THERA-vital is ready, you can start it now by using the console 2.

E Change of radius, without tools
F Change of radius, 3 steps (Option) After the assembling of the optional change of the radius the handle length can be adjusted in 3 steps. By using the change of the radius the mobility capacity of the feet (Hub) can be set according to the individual patient. ❑ Switch THERA-vital off by pressing the ON/OFF button. ❑ Untie the foot-rest with the wrench provided of the handle. Take care: right footrest = right-hand thread, left foot-rest = lefthand thread. ❑ Bring the foot-rest in the desired position and tie the locking screw! ❑ Set the pedal on the other side in the same way. Pay attention to get equal lever lengths (marks on the handles). The smaller the handle length, the higher the power on the legs and joints. ❑ Tie the screw once again after 20 minutes training.
H Calf-Rests (Option)
Adjustment of height
The calf-rests help to support the shanks. They are connected jointed with the foot-rests in order to not hinder the movement of the crank joints. ❑ Adjust the calf-rests regarding the length if needed (2 star-shaped screws). ❑ Fix the shanks with the belts. Patients that have been fixed with the belts in THERA-vital must be supervised.
❑ Untie the star-shaped grip 9 located on the
I Foot Fixing Belts (Option) With this option the feet can be fixed fast, safe and often by your own in the foot-rests: ❑ Open the foot fixing belt in forward direction. ❑ Put the foot in the foot-rest. ❑ Close the foot fixing belt.
B Height adjustment The upper part of THERA-vital with console and therapy grips or upper torso trainer can be set keeping into account the seat height and the physical height of the patient. Set the upper part in a way that makes it possible for you to sit in an upright position.

lower side of the supporting arm for a half a reverse counter-clockwise. ❑ Take out the star-shaped grip to untie the arresting device and place the upper part simultaneously as you wish. ❑ Let go the star-shaped grip and push the supporting arm slightly till the star-shaped grip locks in. ❑ Tighten the star-shaped grip in clockwise direction. Adjust the height of the upper part in a way that your knees can never collide with the console. Set distance at upper part While training the upper body the adjustment is the best possible if the arms at distant pedal position do not get stretched completely. ❑ Untie the star-shaped grip 3 located above the supporting arm for half a turn counterclockwise. ❑ Take out the star-shaped grip to untie the arresting device and place the upper part as you wish at the same time. ❑ Let go the star-shaped grip and push the supporting arm slightly forward or backwards till the star-shaped grip locks in. ❑ Tighten the star-shaped grip clockwise direction.
K Therapy grips
L M Grip-Adaptation
The therapy grips 12 make a variable holding of he grip possible at the upper body training. The therapy grips 12 are easily to deconstruct from the cranks 6 by putting pressure on the lock pin and pulling the therapy grip 12 from the crank. Repeat this at the second therapy grip 12. To put the therapy grips 12 on, push the lock pin on the therapy grip and stick the therapy grip 12 carefully until stud onto the crank 6. Let the lock pin go now. The therapy grip 12 now must engage obviously.
Just a few manipulations and the upper torso trainer is a therapy grip.
J Arm-Rests (Option)
❑ Mount/Dismount the grip-adaptation like
A simple application of the arm-rests 13 is a convenient supplement to offer patients suffering from paralysis, light spasm or stiff hand-/or arm-sections a safe hold. You can fix the underarms in the rests 13 with belts. You can either: ❑ put the arm into the on the therapy-grip 12 attached arm-rest 13 and fix it afterwards or ❑ mount the arm-rests onto the arms first in order to clip them to the therapy grips 12 then. ❑ Arm-rests 13 are fixed by simply pressing them onto the therapy-grip 12 (spring lock).
Lock Handle ❑ Draw down the arresting button 17, turn it round (quarterly turn-round) and let go. ❑ Turn the handle to a horizontal position till the arresting button locks in. To untie the handle draw down the arresting button, turn it round (quarterly turn-round) and let go. Assembling Grip-Adaptation the therapy-grip (see before). The grip adaptations are not designed to get support for your body weight or to carry the device.
N HRM (Heart Rate Measurement) Ear clip THERA-vital has as a standard an Ear clip for heart rate measurement. It serves to control heart rate. It is not designed for making heart rate controlled exercise because of accuracy of measurement (see chapter „Training mode“, page 41).

❑ Plug in the plug at the bottom side of the console. ❑ Preheat the ear and mount the clip at the ear lobe ❑ Fix the cable of the clip at your clothes. Cardio-Pulse-Set (Option) The chest-tape pulse reader (Cardio-PulseSet) has a higher accuracy of measurement and for this can be used for heart rate controlled exercise (see page 41). ❑ Plug in the plug of the receiver on the lower side of the console 2 in a way that the flat side shows to the patient. ❑ The chest-tape has to be moistened on the inside and fixed on the chest (near the heart) in a way that the sender shows to THERA-vital. Several THERA-vital with CardioPulse-Set in a room have to be set in a distance of at least 3 meters in order to not influence each. Before starting the training you can hear the signals of the heart rate as a beeper. So you can check the correct joint of the pulse reader.

Display at console The display at console shows the actual operating state and the settings for the number of revolutions, treading resistance, direction of rotation, arm- or feet-trainer, as well as the duration of the training. In the basic adjustment, following display is shown: Actual treading resistance (see page 35), alternatively capacity condition in Watt. You can change the value before, during and after the training with the buttens + and –. Spasm control (see page 35) active (symbol dark ) or inactive (symbol light). Change the settings with the button next to the display.
Pulse-/ and Capacity display. Appears only when pulse-reader is plugged in. Otherwise there will be shown the actual capacity (Watt).

Selection arm-/leg-trainer (see page 35). The active option is presented in dark colour. This option is only possible if an arm-trainer is installed. Change this adjustment before starting the training by pressing the button next to the display. Stops the training (see page 36)! For interrupting the training press the button once (pause). Pressing the button once again stops the training and shows the results of the last training.

Treading resistance/ Capacity settings You can regulate the power or capacity you want to train 10 with. Following adjustments are possible: Treading resistance (Neuro): In the training mode Neuro (see page 41) you can adjust the treading resistance. The actual treading resistance is shown in 16 steps. The 16th step accords to a maximum power torque of 12 Nm. You can increase the treading resistance by pressing the button +. To increase the treading resistance step by step press the button shortly. You can decrease the treading resistance until zero by pressing the button –. The adjustment will change constantly to maximum/minimum by pressing the buttons + and – for longer time. Power demand settings in Watt (Ortho): In the training mode Ortho (see page 41) you can adjust the training “Power demand ”. The wished capacity gets set by the buttons + and –. THERA-vital regulates the treading resistance in accordance to the number of revolutions.

Number of revolutions Press the button + to increase the number of revolutions. To increase the 34 number of revolutions step 1/min by step push the button shortly. The adjustment will change constantly to maximum/stop by pressing the buttons + and – for longer time. If you increase the number of revolutions before the training THERA-vital will start with the number of revolutions you have adjusted. The automatic program (see page 36 and 39) is not activated. Power demand (Ortho): In training-mode Ortho (see page 41) you can adjust the training-art “Power demand” and you can choose the desired number of revolutions (Isokinetic Mode). THERA-vital regulates the treading resistance automatically in accordance to the expended muscle-power.

Spasm Patients that suffer from spasm or that have very sensible bones and joints are save from to much stress by THERA-vital’s spasm-control. Pressing the button puts the spasm-control on or off. The active spasm-control is shown in dark. THERA-vital stops immediately if the device recognizes a spasm. The display shows “SPASM”. After 5 seconds the device starts

again with a leading number of revolutions reduced by 5 revolutions per minute (see also “Spasm programming” page 39). Please read also the remarks on page 30 “If Spasm occurs”.

Direction of rotation The actual direction of rotation is shown by the dark symbol. If you press the button the direction of rotation changes. The direction always changes with a soft stop and a soft a start. The rest of the parameters (number of rotations, motor power, arm-/feettrainer, spasm control) stay as they were set. Leg trainer/upper torso trainer You can either activate the leg trainer or the upper torso/arm trainer. The active trainer will be symbolised in dark. Press the button before starting the training, press it in a pause to change between the feet- and the upper torso trainer. The button is not active during the training or if there is no upper torso trainer assembled.
Get-in-help Use the get-in-help to bring the pedals one after the other into a favourable position. ❑ Push the button shortly and wait until the get-in-position of the foot-rest is reached (feet-rest is in a lower position). ❑ Put in your foot in the lower foot-rest. ❑ Push the button once again and wait until the second get-in-position is reached. ❑ Fix the first leg now into the upper foot-rest and put the second foot into the other footrest afterwards. ❑ Push the button once again and wait until the first get-in-position is reached. Fix the second leg now.
STOP

START/ STOP
In the stand-by mode THERA-vital will be activated and the display will be lightened by pressing the START button. Pressing the button once again will start the training. The device starts with 10 revolutions per minute. If you stay passively the number of rotations will be adjusted after 1 minute automatically to 25 revolutions per minute. You can change to active training on every point of the training by using muscle power that exceeds the leading number of rotations of the motor. A training unit can be stopped by pushing the STOP button once (pause) and continued with START. Pushing the button STOP two times during a training, it will stop the same. The display will show the training results (see page 41.

Subprogram (for professional user) THERA-vital contains different programs of training which are closely related to the needs of typical patients. For each group of patients can be adjusted training-parameters in the subprogram: ❑ Neuro (basic setting): Training-programs for patients with neurological diseases like Hemi-, Paraplegic, MS, … ❑ Ortho: Training-programs for orthopaedic patients like after knee- or hip-surgery. ❑ Cardio: Training-programs for heart- and circuit-patients, who are meant to train at a certain heart rate. The whole menu-selection is described on page 38.
Navigation in the subprogram To open the subprogram push both minus-buttons at the same time.
The entries Neuro, Ortho, Cardio and technique will appear on the display. The function of the buttons will change:
Actual area in the subprogram Down/Less This button will lead you further down the options. The chosen point in menu will be put into dark (inverted). Concerning points in the menu that need an input of worth (i. e. training time) the worth will be reduced.
Stop/dismiss changes This button will lead you further in the menu selection. With ✔ saved changes will be used further on, changes that are not yet saved with ✔ will not be saved.

Description of the functions of the training Training time The training time can be preset from 1 to 60 minutes in every training program. Standard is 30 minutes.
The sensibility is also closely related to the radius of the handle (see pages 31, 32) and the engine power (see below).
Automatic change of direction of rotation THERA-vital changes every 4 minutes in passive training the direction of rotation. While training actively, there will be no changing of the direction of rotation.
Spasm program The spasm-feature of THERA-vital sets in which direction the device starts again after a spasm. ❑ Choose the adjustment as follows: Forward: when having the tendency for stretch spasm; Backward: when having the tendency for bowing spasm.
Sensitivity of spasm control This feature enables you to set the borderline for the sensory of THERA-vital to notice a wrong reaction of the patient that will cause the engine to stop, in order to minimize any possible overstraining. ❑ For patients tending to show any symptoms of spasm choose the adjustment as follows: fine for patients usually reacting sensibly. With this adjustment usually a low resistance is sufficient to stop spasm. medium for patients reacting less sensible. rough for patients with a low risk for spasm. The device will stop with a high resistance.
Automatic Number of Revolutions-Adjustment At the active training the adjustment of the number of revolutions automatically adjusts to a higher capacity level of the patient, if the automatic adjustment of the number of revolutions is set “on”. If the patient works with a number of revolutions of 30 rpm instead of the preset number of revolutions of i.e.15 rpm, THERA-vital will adjust after approximately 10 sec the preset number of revolutions on 25 rev./min. If the patient afterwards gets passive again, THERA-vital keeps working with 25 rev./min. This way the patient is working towards his own individual number of revolutions and when showing passiveness will get trained with a convenient number of revolutions.

Motor power The preset motor power has a stronger effect on the patients legs if a small pedal radius is chosen as if a big radius is chosen. ❑ Please set the motor power as follows: high when pedal radius is big (100 % motor power), medium when pedal radius is medium (approx. 80 % motor power), low when pedal radius is small (approx. 60 % motor power). Small pedal radius and adjustment of motor power on “big” can effect very big powers on the patient.

Active muscle-support This function helps patients that are unable to get a smooth rotation with just one leg or arm. The motor supports the active side by periodically doing the work on the in-active side needed. The feature works like a flywheel at mechanical training devices, only that THERA-vital does not have to get driven. Any observers will not notice that the patient only works one-sided. Is the patient able to work with both sides the active muscle-support is not to notice.
Bio-Feedback Bio-Feedback serves to give the patient a result of the movement process during the exercise. In the upper part of the display you will find the following values: ❑ actually set treading resistance/set power demand, ❑ actual number of rotations of the pedals, ❑ remaining training time, ❑ distance, ❑ if HRM reader is plugged in, the actual heart rate, or ❑ the actual achieved capacity of the patient when training actively. Bio-Feedback is only possible during legtraining and in direction of rotation forward ( ). Sym-Bar/Sym-Road: These both ways of visualising feedback show how symmetric the active training is, that means how big the differences in capacity are on the right and on the left leg. The result can be falsified if the pedal radius is adjusted differentely (see pages 31, 32) or the patient is not seated in a middle position to the device. The Bar-Diagram shows the activity in the left/right leg. In summary, there will be always 100 %.

The %-value of two training units are only comparable if the radius of the pedals and the maximum motor power (subprogram) are set the same. This representation is not meant for scientific evaluations! It is meant to show the patient whether he is during his training very soft or very tied or if the muscle tone was reduced.

Display of capacity At the active training the capacity can be shown as capacity in Watt or as energy consumption in kcal by the bio-feedback. This adjustment is only possible in the training mode Ortho. Training mode There are 4 training modes, according to the group of patients: ❑ Setion of resistance (Neuro): the set resistance will be constant during the whole speed range. A higher treading frequency means a higher capacity. ❑ Power demand set (Ortho): The patient trains with a certain capacity in Watt which is given. The device regulates the resistance independentely of the number of rotations, so that the patient always has to achieve the preset capacity. ❑ Speed demand (Isokinetic mode), (Ortho): The device regulates the resistance in a way that the patient will always train with the same number of rotations when training actively. Exceeds the patient the number of rotations the device will increase the resistance. Falls the patient short of the set number of rotations the device reduces the resistance. ❑ HRM control (Cardio): The device regulates the resistance in a way that the patients heart rate will always remain on the preset frequency.

Evaluation of the training In the end of the training an evaluation is shown. These results are only possible if the training lasts at least one minute and it was started with the button START.

Accustic stop-function (Voice Control)
Basic adjustment for the training
This feature makes it possible to stop THERA-vital in a danger situation by a long, loud call that lasts 3 seconds. The acoustic stop-function is always active. You can adjust the sensitivity of the microphone which is included in the console in the menu technique/sensitivity of microphone. ❑ Adjust the sensitivity as follows: <> if very loud noises occur in the surrounding area <++> if normal noises occur in the surrounding area <++++> if little noises occur in the surrounding area ❑ Check the correct adjustment in a practical trial. To continue the training push the button START after an acoustic stop.
In condition of delivery the settings like on page 38 in the column standard described are programmed. If you change anything in the subprogram these changes will remain even if you switch the device off or take it off the socket. The next training can be done in the same conditions. If you want to change the basic adjustments into the standards again: ❑ Push the STOP button and keep it pressed. Push now the minus buttons simultaneously. Now the standards as described on page 38 are valid again. The adjustments in the area “technique” remain.
