Sitter Select

Before You Begin The Posey Sitter Select is an electronic monitoring system designed to help ­prevent falls in settings where continuous surveillance of admissions is not ­possible. Each system consists of an alarm unit, with an adjustable cord with magnet that attaches to the patient and input for an optional bed or chair ­sensor. As the patient attempts to rise and disengages the magnet or removes weight from the sensor, the alarm is activated. When the Sitter Select is ­connected to the nurse call station, the room light and the nurse call station signal are also activated. In addition to providing an immediate warning of a potential fall, the Sitter Select can be used as an assessment tool in determining appropriate fall ­management protocol for a particular admission. Prior to using the Sitter Select, each facility should create policies and procedures regarding its use. Some of the issues to be addressed are:

Indications (these may include): • Individuals with diminished cognitive or mobility skills, i.e. the frail, disabled, senile or those with neurological deficits. • Patients receiving medications that may cause disorientation, drowsiness, dizziness or ­frequent urination, i.e. diuretics. • Any admission (new or existing) with a history of falling. • Individuals who are restless or prone to getting up in the middle of the night, i.e. incontinence, those with nocturnal voiding patterns. • Any new admission where the patient assessment documents fall risk, i.e. the elderly, ­medicated, those with functional or cognitive deficits. • Patients requiring mandatory bed rest.

Contraindications: The Sitter Select should never be used as a sole surveillance device for agitated, ­combative or suicidal admissions, or for those at extreme physical risk for a life-threatening fall. In these cases, the Sitter Select, along with a more intensive fall ­prevention measure would be required.

Response: Each facility should determine the appropriate protocol for responding to fall monitor alarms. This protocol might include calling over the intercom, telling the individual to wait for ­assistance and then sending staff immediately to the room. Be sure to put notification next to the room light at the nurses’ station to identify high fall risk patients. This will help alert the nursing staff to respond immediately when the call signal is activated.

Nursing Surveillance: Each policy should address the frequency of visits by nursing staff to confirm the patient/resident is safe. In addition, these visits should include periodic verification that the Sitter Select is ­functioning properly.

To verify the Sitter Select is working properly: Magnet Activation: 1. Make sure the LED light on the unit is lit, indicating the unit is “ON”. 2. Verify that the batteries are fresh. If you hear an intermittent “chirp”, the batteries are low and need to be changed. 3. Check that the audible alarm or voice message is functioning properly by removing the ­magnet and activating the alarm. If the alarm is set to “Voice Only” or Voice and Tone” mode and the voice does not play, re-record the message. If, after recording a new message, the alarm still does not properly activate, remove the batteries for a minimum of 1 hour to allow the alarm unit to reset to the factory default settings. 4. Ensure the nurse call interface, when used, is connected properly and activates when the magnet is disengaged from the face of the unit. 5. Check the cord for possible entanglement. Be sure it is the appropriate length to ensure safe monitoring of each patient/resident.

Sensor Activation: Connect a Posey Sensor to the control unit and place weight on the sensor. Note: When a sensor is plugged into the unit, the sensor takes precedence over the magnet. 1. Disengage the magnet while monitoring a patient/resident with a sensor pad to ensure proper alarm activation. 2. Make sure the LED light on the unit is lit, indicating the unit is “ON”. 3. Verify the batteries are fresh. If you hear an intermittent “chirp”, the batteries are low and need to be changed. 4. Check to make sure the audible alarm and/or custom voice recording is functioning properly by lifting weight off the sensor pad to activate the alarm. If the alarm is set to “Voice Only” or Voice and Tone” mode and the voice does not play, re-record the message. If, after recording a new message, the alarm still does not properly activate, remove the batteries for a minimum of 1 hour to allow the alarm unit to reset to the factory default settings. 5. Ensure the nurse call interface, when used, is connected properly and activates when weight is removed from the sensor. To avoid possible injury or death: Do not use the Posey Sitter Select if the audible alarm does not sound or the nurse call ­system does not activate upon bed exit. Verify system before each use. Replace batteries if the audible low battery warning sounds. The Posey Sitter Select is an electronic device. If the unit is subjected to severe mechanical shock, such as dropping the unit, or is submerged in liquid, the unit may stop functioning as designed. After each incident, you must verify that the unit is working properly as outlined above.

Benefits of the Sitter Select The Posey Sitter Select is a simple addition to your fall management program. The restraintfree alarm notifies you when a patient/resident attempts to rise from a bed or chair without assistance. The voice option allows you to record a personalized message to remind the patient/ resident to call for assistance, and can be used in place of the tone alarm, if desired. The three easily adjusted volume settings, four alarm modes and five tones allow you to tailor the monitor to your patients’ and facility’s needs. The Nurse Call interface allows you to silence the in-room alarm to enable a roommates undisturbed sleep. The multiple light indicators allow staff to see, at a glance, day or night, that the unit is working correctly. Backed by Posey’s limited lifetime guarantee, the Posey Sitter Select offers total assurance in one easy-to-use package.

The Posey Sitter Select offers caregivers a choice between sensor activation and magnet ­activation all in one easy to use, compact fall monitor. • In the magnet activation setting, when the magnet is disengaged from the face of the unit the alarm will sound. • In the sensor activation setting, when weight is removed from the sensor the alarm will sound.

Inserting/Changing the Batteries The battery-operated Posey Sitter Select is portable and long lasting; estimated battery life is 54 hours of continuous alarm. Actual battery life is dependent on the frequency of the alarm derivation and on the mode, tone and volume selected, as well as the freshness of the batteries. Your new Sitter Select is shipped to you with 4 fresh “AA” batteries.

Turning alarm on/off: There is no on/off switch on your Posey Sitter Select. This feature is intention­ally excluded to prevent resident tampering and/or staff from inadvertently turning units off. To store your Posey Sitter Select, you may leave the batteries in the unit. Though the lights continue to blink, only a small amount of power is being used and batteries should remain fresh for extended periods. If preferred, you may leave the batteries in the unit and simply slide the battery door open just enough to disconnect batteries and turn the power off.

To insert batteries: 1. Press down on arrow, slide ­battery compartment door open and flip it up (see Fig. 1). Note: The door does not ­separate from the unit. 2. Insert four (4) new “AA” ­batteries as pictured inside the battery compartment (see Fig. 2). 3. Flip the battery door down and slide closed. Note: You must push down on batteries while closing the door.

If the unit has lost power for more than four (4) minutes, all settings, including voice message will revert to the factory default settings. If power has been absent for less than four (4) minutes, the unit will come ON with the same settings and voice message selected before power was interrupted. NOTE: You will hear an intermittent “CHIRP” when new batteries are needed. Fresh batteries should be inserted immediately and should last approximately 40 days. Do not mix old and new batteries, or battery brands. Always install four (4) fresh batteries of the same brand. Change the batteries immediately upon hearing the audible low b­ attery warning. After battery change, check alarm function and verify all selected settings before each use, ensuring the alarm unit is securely positioned and functioning properly.

Setting Alarm Mode The Posey Sitter Select has four alarm modes that allow you to select the appropriate alarm signal for your facility and each patient/resident. An indicator light shows which mode is ­currently in use (see Fig. 3). VOICE & TONE

To Change/Select Mode: 1. Press MODE button on the right side of the unit to cycle through the choices (see Fig. 4). 2. The blinking light indicates which mode is currently selected. MODE Indicator lights

NOTE: VOICE only and MUTE are not available unless the Nurse Call cable is connected to the alarm unit. With Nurse Call Interface, all mode options may be selected.

Setting Alarm Tone There are five (5) tones available. This allows you to differentiate between patients/residents and from other equipment alarms.

To Select Tone: 1. Press the TONE button on the right side of the unit (see Fig. 5) to scroll through the tone selections. 2. Each time you press the TONE button, the selection changes and a two (2) second sample of the tone is played. 3. Continue pressing the TONE button until you hear the desired tone selection. The last sample played will be the tone selected for use.

Adjusting Alarm Volume There are three (3) alarm volume settings available; low (80dB), medium (90dB), and high (103dB). This allows the caregiver to set alarm volume to suit facility and/or patient needs. All volume settings are within OSHA standards. For maximum notification of caregiver, use the highest (loudest) volume setting. Always test alarm to make sure staff can hear the alarm sound before leaving the patient/resident alone.

To Change Volume: 1. Press the VOLUME button on the right side of the unit (see Fig. 6) to scroll through the volume selections. 2. Each time you press the VOLUME button the selection changes and a two (2) second sample of the volume is played. 3. Continue pressing the VOLUME button until you hear the desired volume setting. The last sample played will be the volume selected for use. Never place the unit closer than one-foot from the ear. Do not reduce the alarm ­volume if patient can travel out of hearing distance of staff by wheelchair or other means.

Setting A Time Delay A time delay of 0, 2, or 4 seconds may be selected. The time delay option is available ONLY when a sensor is in use. This time delay is the acceptable time interval that can elapse AFTER the patient/resident is off the sensor pad BEFORE the alarm is activated. Each time delay should be set according to each individual patient’s needs. The time delay should be long enough to allow movement in bed without setting off an alarm, yet still give sufficient warning to nursing personnel of an attempt to ambulate unassisted. The time delay switch is located inside the battery compartment. This minimizes tampering and unauthorized setting changes.

To Set A Time Delay 1. Open the battery compartment and remove the batteries 2. Slide the toggle switch (see Fig. 7) to the desired delay setting, 0-2-4. 3. Re-insert the batteries and close the battery compartment door. NOTE: The Posey Sitter Select automatically resets. If weight is removed from the sensor and reapplied BEFORE the full time delay has elapsed, the time delay will be reset for the full time interval and the unit will resume monitoring. If weight is removed from the sensor for the full time delay interval, the unit will alarm. When weight is reapplied to the sensor, the time delay will be reset for the full time interval and the unit will resume monitoring.

Recording a Message

The voice feature of your new Posey Sitter Select allows the caregiver or family member to record a personalized message of instruction that will “play back” to the patient/resident when the alarm is activated. This is ideal in situations where sounding an alarm may agitate residents or disturb a roommates’ sleep, or in situations where English is not the residents native language. NOTE: The minimum length of the voice record message must be 2 seconds and the maximum length is 10 seconds. Following are examples of voice messages you may record: “Mom, remember you have broken your hip. Please wait for the nurse to help you.” Or in Spanish “Mama, recuerde que tiene la cadera quebrada. Por favor espere que la enfermera le ayude.”

To Record A Message 1. Press and hold the RECORD button on the right side of the unit (see Fig. 9). 2. Wait for the beep before speaking. 3. In a normal voice, talk into the microphone aperture labeled “MIC” on the front of the unit (see Fig. 10). Note: The volume adjustment will not change the recording volume. The volume at which you speak and your distance from the aperture determines the message ­volume. To increase the volume, speak in a louder voice closer to the mic. 4. The recording will stop when you release the button or when time expires. If you exceed the allotted time, you will hear a beep indicating recording has stopped. 5. After recording a new message, either disengage the magnet or remove weight from the ­sensor to play back the message. 6. Listen to ensure clarity and volume are acceptable. 7. If no message has been recorded or power has been removed from the unit for more than four minutes, the message played will be the factory default message. Voice & Tone

Always test voice message after recording to make sure it is active. Press and release the sensor, or remove the magnet to hear voice. If voice does not play, re-record message. If alarm does not sound properly, reset to factory settings by removing the batteries for one hour. Replace batteries and record a new message.

Setting The Unit On Hold The HOLD feature of your new Sitter Select is accessible only when a sensor is in use. The HOLD feature allows the patient/resident to be away from bed (i.e. therapy, toileting, socializing) without the need for the alarm to be powered down. The alarm automatically reactivates when the patient returns to bed. This ensures ­continuity of care and monitoring despite ­multiple caregiver involvement.

To Place The Unit On HOLD 1. Press the HOLD button on the right side of the unit (see Fig. 11) until the HOLD light on the front of the unit is flashing (see Fig. 12). 2. As soon as the unit is placed on HOLD, you will have a 30-second interval where the unit will not respond to weight on the sensor. This is to prevent the unit from turning on and off while the patient makes his way to the edge of the bed in preparation to ambulate. It is recommended that you position your patient towards the edge of the bed with his legs over the side before pressing the HOLD button. 3. At the end of the 30-second interval, if weight is present on the sensor, the unit will activate and begin monitoring. You will hear a “Chirp,” indicating unit is activated. 4. To take the unit out of HOLD you may • Apply weight to the sensor OR • Press the HOLD button To avoid possible injury or death: Verify the green power light is lit and the unit is in the appropriate monitoring mode before leaving patient/ resident unattended.

Connecting the Nurse Call If desired, your new Posey Sitter Select may be connected to the facility nurse call system to provide additional notification to the caregiver. With nurse call interface, the alarm unit may be silenced in the patient’s room to permit roommate’s sleep or set so that a voice message only plays instead of an alarm tone.

To Connect The Nurse Call: 1. Insert the Nurse Call Cable into the receptacle labeled “Nurse Call” on the left side of the unit (see Fig. 13). 2. Plug the other end into the wall jack of the nurse call panel. A special adapter may be required. Please refer to pages 31-34 of this manual or Contact Posey Customer Service at 800-447-6739 for additional information. NOTE: If the cable is disconnected from the alarm unit while in the Voice Only or Mute modes, the alarm will automatically switch to Voice & Tone. This safety feature is to ensure caregiver notification of patient/residents attempt to rise. NOTE: To utilize a nurse call button simultaneously, you must use a “Y” adapter (Cat. No. 8235S) to enable your system to accept both the Posey Sitter Select and the nurse call button. “Y” adapters are available from the Posey Company. Please contact Customer Service at 800-447-6739. Always test the alarm function and nurse call activaterly. DO NOT plug the nurse call cable into the alarm unit without connecting the other end to your facility nurse call system. If not properly connected to the unit AND the nurse call system, the nurse call will NOT activate when the unit alarms.

Sensor Pads Under Mattress Sensors 1. Select the appropriate sensor for your mattress. Lightweight sensors are for mattresses that weigh 38 – 42lbs. Heavy weight sensors are for mattresses that weigh > 42lbs, 2. Place the sensor pad under the mattress directly on top of the bed frame in one of two positions. A. For patients/residents at extremely high risk of falling (or when using restraints), position the sensor pad at the shoulder area (see Fig. 14) with a short time delay so that as the patient/ Sensor at Mattress resident sits up, the alarm is activated. shoulder Sensor Bed position under B. Secure the pad to the bed frame using the hook-and-loop straps or cable ties, depending on the style of bed. 5. Connect the sensor to the alarm unit by plugging into the jack on the left side of the unit labeled “sensor” (see Fig. 15). 6. Disengage the magnet from the front of the alarm unit and store in a safe place. 7. Magnet Storage – Peel the backing off the metallic plate and affix to the pre-selected storage spot. The Sitter Select is designed to monitor your residents using either the magnet ­activation or the sensor activation. The two may NOT be used simultaneously. The Sitter Select is programmed to provide a “failsafe” sensor alarm. When using the sensor activation with the magnet disengaged, if the sensor is disconnected or the wire breaks, the alarm will sound to notify the caregiver of possible interruption of monitoring. If the magnet is engaged on the front of the unit when using a sensor, the magnet will override the sensor ­failsafe feature. Therefore, it is VERY important that you ­disengage the magnet from the front of the unit when using a sensor.

Over Mattress Sensors 1. Select the appropriate sensor for your patient. Disposable 30-day sensors for short-term stays or 6-month sensors for longer term admissions. 2. Remove the covering from the adhesive tape on the bottom of the sensor if you are using adhesive tape to secure the sensor. 3. Position the sensor on top of mattress, across width of the bed in one of two positions (see Fig. 18). A. Directly under the patient’s shoulders; alarm will sound when the patient sits up. B. Connecting the Cord & Magnet to a Resident The Posey Sitter Select may be used to monitor a patient/resident in bed, chair or on the toilet. The Sitter Select is equipped with an adjustable cord to allow the caregiver to select the ­appropriate safety zone and mobility level for each patient/resident. It is the caregiver’s responsibility to ensure that the cord length selected ensures safe and effective monitoring.

To Connect The Sitter Select To Your Patient/Resident: 1. Determine the desired “safety zone” including range of mobility for patient/resident. 2. Adjust the cord length with the slide adjuster to desired length. Cord adjusts from 31 inches to 60 inches. 3. Attach the clip near the shoulder, on a piece of clothing the patient is not likely to remove. The point of attachment should be out of the patient’s reach (see Fig. 22a and b).

Posey Sitter Select and Magnets: Posey bed exit alarm pull cord magnets are strong enough that DIRECT CONTACT with a pacemaker could affect pacemaker function. Since pacemakers are inserted underneath the skin, direct contact of pull cord magnets with a patient’s pacemaker is highly unlikely. At 1 ½” the Posey pull cord magnet strength is .8 Gauss. This is 28 times lower than the force required to interrupt the pacemaker device and not enough force for the magnetic force to hold the magnet on the pacemaker site. Based on consultation from two pacemaker manufacturers, the recommendation is to keep the magnet 1 ½” away from the pacemaker/defibrillator insertion site and do not tape the magnet over the pacemaker site. To Avoid Possible Injury Or Death: Pacemakers: Magnet contact CAN affect pacemakers if brought within 1½ inches of the patient’s pacemaker insertion site. Magnet contact could cause the pacemaker to go into magnetic mode prompting the pacemaker to increase or decrease speed. Defibrillators: Magnet contact CAN STOP THERAPY! If the patient is in ventricular spasms, the magnet suspends the signal and can cause the defibrillator not to respond and apply the needed therapy. This could cause the patient to expire. Do not adhere any type of magnet to pacemaker site. This device may not be suitable for use with all “high fall risk” patients. Healthcare personnel should not use this device as a substitute for maintaining routine visual monitoring.

Beds and wheelchairs vary widely; therefore it is impossible to assign a ­recommended cord length without taking the individual patient and product application into consideration. It is the caregiver’s responsibility to select the appropriate cord length for each patient/resident. When selecting the appropriate cord length for an individual, take all of the following into account: • The cord should be long enough to allow the patient/resident moderate movement to enhance sleep comfort, yet short enough to cause the alarm unit to alert the caregiver that the patient/resident has left the predetermined range of mobility. • The extra distance from the head of the bed to wall should be factored in if the monitor will be mounted on the wall. • If the monitored patient/resident’s bed is elevated and lowered, the cord length should be changed to accommodate this movement. If the cord is too short when the mattress is ­elevated, the monitor may deliver a “false alarm.” If the cord is too long when the bed is lowered, the patient/resident may have more opportunity to move beyond the predetermined range of mobility. • Check to ensure the cord is free to pull straight from the monitor, unimpeded by pillows or bedding, and that it is not entangled in any way in the side rails or headboard of the bed. • Always check to be sure the alarm unit is properly mounted. When used in conjunction with side rails, be sure the cord length accommodates the use of the side rails, and is not subject to entanglement.

Attaching the Bed Mounting Bracket The Posey Sitter Select is packaged with two separate mounting brackets, one for wall or chair mounting and one for bed mounting. Both brackets are designed with a “locking” mechanism that secures the alarm in position and helps prevent lost inventory. For additional brackets please call Posey Customer Service at 800-447-6739.

For Headboard/Footboard Attachment: 1. Use wire bracket (Cat. No. 8276), included in your package, for headboard/footboard mounting (see Fig. 23). 2. Slide the alarm unit on the bracket from the top down until it locks into place (see Fig. 24). 3. Attach the bracket to the headboard or footboard by pulling the wire away from the alarm unit to create an opening wide enough to fit over the headboard/footboard. Slide the bracket onto the bed and push down to ensure a snug and secure fit. The indicator lights should be clearly visible to the caregiver. NOTE: The Sitter Select comes with a standard size bed bracket that fits boards 1/2” – 2” thick. A large bed bracket that fits boards up to 3” thick is available upon request (Cat. No. 8277). 4. To remove the alarm unit from the bracket, gently push in on the bracket release lever while sliding the alarm unit up and out (See Fig. 25). Always check mounting before each use, ensuring the alarm unit is securely positioned and functioning properly. The indicator lights should be in plain view and monitored regularly. Slide Alarm Out

Attaching the Posey Sitter Select to a Wall 1. Identify a location that is out of the patient/resident’s reach, and will allow unobstructed view of the indicator lights. 2. Use the wall/chair bracket (Cat. No. 8301, see Fig. 26). 3. Select the attachment method – hook-and-loop adhesive strips or screws. A. Hook-And-Loop Method i. Place one (1) strip of the included hook-and-loop on the flush side of the main bracket. ii. Place the other hook-and-loop strip with adhesive on the wall where you want the alarm unit. iii. Firmly attach the hook-and-loop on the main bracket to the hook-and-loop strip on the wall. B. Screw Attachment i. Position the wall-mounting bracket with the flat side against the wall (see Fig. 26). ii. Using the wall-mounting bracket as a guide, mark the spots to insert the anchors into the wall. CAUTION: Be sure the selected mounting area is safe for drilling. iii. Drill holes where marked and insert anchors. iv. Position bracket over holes and insert screws. 4. Slide the alarm unit on the bracket from the top down until it locks into place. 5. To remove the alarm unit from the bracket, gently push in on the release lever while sliding the alarm up (see Fig. 27).

The alarm unit should be placed out of the patient/resident’s reach to prevent tampering. Always check mounting before each use, ensuring the alarm unit is securely ­positioned and functioning properly.
