Introduction
TRI-FLEX II BED OPERATIONAL MANUAL Overview
This bed is intended for use whenever a patient’s weight and/or overall size exceeds existing unit limitations or patient’s weight exceeds that which the staff can care for without reducing the quality of service and endangering the safety of patient and staff. This patient weight usually includes anyone over 400 lbs. (181.4 kg.) but not to exceed 1100 lbs. (500kg.). The Bariatric bed is multi-purpose capable. The patient surface can be raised or lowered to facilitate patient care. The bed has trendelenberg and reverse trendelenberg function in the pendant as well as “Cardio-Chair” positioning. The Bariatric system can also facilitate patient transport. The two foot end casters contain steering lock ability which provides directional control during transport. NOTE: This manual covers the use and functions of the basic bed. Instructions for various options and accessories such as scale systems and mattress systems will be covered in supplemental literature provided with those accessories.
Operating Precautions
Before operating this bed ensure that you have read and understood in detail the content of this manual. It is important that you read and strictly adhere to the safety information contained in this manual. This product is intended for indoor use at room temperatures with intermittent operation consistent with the stated duty cycle of the device. Any reference to a side of the bed is from the patient’s view laying in the bed on their back.

Pendant Controls
The pendant control of the bed controls all bed functions. The pendant symbols are def ned as follows: HEAD GATCH - UP/DOWN adjusts upper body for patient comfort KNEE GATCH - UP/DOWN adjust knee for patient comfort FULL BED HI/LOW - UP/DOWN adjusts bed height for transfer and/or other patient care. return to full down when patient is unattended TRENDELENBERG - UP/DOWN adjust full bed for trendelenberg function HEAD/KNEE - UP/DOWN both head and knee gatch work simultaneously

Important Safety Information WARNING: All persons with access to the bed must read the entire manual before using the bed. Failure to do so could result in improper use which could damage the bed or injure persons. WARNING: 1,100 lbs. (500 kg.) patient use WEIGHT LIMIT. Use of bed with persons above this weight may cause damage to bed and persons. WARNING: To transport a patient in bed the bed high-low must be in its lowest position with the frame resting on the transverse caster bars (see f gure 1). Failure to do so could result in compromising the structural integrity and stability of the bed which could damage the bed or injure persons. See transport section of the manual.

WARNING: Possible fire hazard exists when bed is used with OXYGEN administering equipment other than a nasal mask or 1/2 bed length tent. Oxygen tents should not extend below the mattress support level. WARNING: AVOID CONTACT WITH INJURED SKIN. It is not intended for this device to come in contact with injured skin. Ensure that injured skin is properly protected. WARNING: The Bariatric electronics are not designed to be operated in environments containing FLAMMABLE ANESTHETICS. Possible f re hazard exists. WARNING: Keep the SIDERAILS in a raised position while patient is on the bed to prevent patient from accidentally falling off the bed. WARNING: Bed should be left in the lowest position when unattended in order to reduce the risk of injury due to falls while getting into or out of the bed, or while lying on the bed. WARNING: If PATIENT RESTRAINTS are used they must be attached to some point on the bed that articulates when the gatches are raised and lowered. Failure to do so could result in the restraints injuring the patient.

Important Safety Information WARNING: To maximize the full scope of UL-60601 / IEC-60601-2-38, this bed is intended for patient use in ONLY the 48” or 54” width conf gurations. Within the current standard, patient and caregivers are safely removed from pinch points and moving components which could cause harm. Conf gurations other than described reduce the distance to these points which could result in severe personal injury or equipment damage. WARNING: The mattress intended for use with this bed is 7 inches thick. Use of a mattress taller than 7 inches will not be in compliance with the UL-60601 / IEC-606012-38 rating of the bed. A taller mattress reduces the distance between the top of the side rail and the top of the mattress. Serious injury or equipment damage may occur if the patient goes over the top of the raised side rails. WARNING: Since 1999 the U.S. Hospital Bed Standard IEC-60601-2-38 requires the height of the top edge of the side rail above mattress without compression to be at least 220mm or 8.6” CAUTION: The POWER CORD must be positioned so as to prevent being walked on or rolled over by the bed casters or interfering with moving parts of the bed. Keep the power cord plugged into a wall outlet. CAUTION: Hang the hand CONTROL PENDANT on the side rail when not in use. Run the cord under the bed frame and keep it clear of moving parts to prevent possible damage to the cord. WARNING: When the bed is in use, the CASTERS must be positioned so that the locking tabs are locked and pointed towards the foot end of the bed. Failure to do so will cause damage to the bed and possible personal injury. Caution: Bed must be positioned 4”- 6” from wall without a trapeze and 17” - 19” from wall with a trapeze when in the Trendelenberg position. •
Elevating the head gatch facilitates easier breathing for heavier patients. Consult a doctor.

If needed, orthopedic equipment can be attached to the bed.
Power and Battery Operation The electronics system of the Bariatric bed operates from a low voltage power supply located under the head of the bed. This control system also includes a battery back up system to continue operation of the bed in the event of AC mains failure or during transport. The back up battery will provide the opportunity for several adjustments for patient comfort or transport during power outage, however it is not intended for extended use. In the situation of an extended power outage it is recommended that the patient be removed from the bed.

Assembly/ Disassembly Instruction NOTE: MOVE BED TO AN OPEN AREA LARGE ENOUGH TO UNFOLD THE BED. IF YOUR BED IS NOT EQUIPPED WITH THE OPTIONAL SCALE SYSTEM, PLEASE DISREGARD THE INFORMATION PERTAINING TO THE SCALE SYSTEM. UNFOLDING THE BED FOR USE 1. Set the head and foot boards aside to be assembled later. Also remove the two turnbuckles and set aside for later use. 2. WARNING: Lock the four transport casters to prevent the bed from moving when you unfold the head and foot sections as movement could cause injury. The locked casters should be facing outward (caster lock out) towards the head and foot ends of the folded bed (see f gure 2).

3. Remove the “L” shaped foot end transport latch located on the transport pole. (see f gure 3) 4. Gently lower the foot end section of the bed. (see f gure 4) WARNING: If the foot end or head end of the bed is allowed to drop while being lowered, injury to persons and/or damage to the bed could occur. 5. Make sure that the black woven safety strap located at the head end of the bed is securing the head gatch patient support surface to the base (see f gure 5) . WARNING: Unexpected movement of the patient support surface and possible injury could occur if the safety strap is not in place. CAUTION: Loosen this safety strap before operating the bed.

Assembly/ Disassembly Instruction 7. Gently lower the head end section of the bed. WARNING: If the foot end or head end of the bed is allowed to drop while being lowered, injury to persons and/or damage to the bed could occur (see f gure 6). 8. Release the black woven safety strap located at the head end of the bed and remove the Tool (see f gure 7). 9. Pin the turnbuckle in place at the connection points below the head and foot sections of the bed. (DO ONE SIDE AT A TIME) Using the Tool provided place one end through the center of the turnbuckle. Look for the dots on the end of the screws and reference them to the marks on the turnbuckle body. (see f gure 8). As you adjust the turnbuckle body the dots on the screws will move toward each other when tightening and away from each other when loosening. Figure 6 Observe the correct movement inward as you install this piece. Observe the correct movement outward when removing this piece. WARNING: Install and completely tighten one turnbuckle and then install and tighten the other side. Failure to properly install the turnbuckles could result in injury to persons and/ or damage to the bed. 10. Store the Tool in Velcro holder with black safety strap as in (f gure 7).

11. CAUTION: Position and lock the four casters (located at the corners off the bed) with locking tabs facing the foot end of bed. Failure to do so will result in damage to the casters during hi-low use. 12. Plug bed power supply into an AC outlet.

13. Raise bed hi-low approx. 5 inches to take weight off of the center transport wheels.
14. Remove the pull pin securing the central transport system. Pivot the axle inward and up. Align the holes to where the casters are tucked up under the bed and replace the pull pin.

Assembly/ Disassembly Instruction 15. Raise the head gatch and store the central transport pole in it’s holder under the head gatch (see f gure 9) 16. Lower the Hi-low system. 17. Install the complete mattress system with side bolsters. 18. Install head and foot boards into rectangular brackets at the ends of the bed. 19. Hang the scale readout assembly on the foot board and connect the cable. FOLDING THE BED FOR TRANSPORT 1. Disconnect the scale display cable, lift the assembly off of the foot board and set aside. 2. Remove the head and foot boards and set aside. 3. Remove mattress system. 4. Pull the head pullouts to the outward position and lock in Figure 9 place. Raise the head side rails and lock in place. Push the center and upper foot pullouts to the inward position and lock in place. Pull the lower foot pullouts approximately half way between the inner and outer positions. Rotate the foot side rails to the down or open position. If the bed has fold down foot rails they must be removed to fold the bed. 5. Raise the head gatch and remove the central transport pole stored under the gatch. Install the transport pole vertically in the center of the bed and pin it in place . Lower the head gatch and foot gatch. 6. Raise the bed hi-low approx. 5 inches to allow the transport wheel assemblies room to pivot down. 7. Remove the pull pin securing the central transport wheel assembly. Pivot each assembly down and lock in place with the pull pin. 8. CAUTION: Lock the four transport casters to prevent them from allowing the bed to roll away from you when you lift the head and foot sections. The locked casters should be facing toward their respective end of the bed (see f gure 2). 9. Press the hi-low “down” button on the pendant to allow the weight of the bed to rest on the transport casters in the center of the bed. 10. Unplug the AC power cord. 11. Release the black safety strap and remove the Tool. 12. Loosen and completely remove the two turnbuckles with the wrench. Just as in step 9 of unfolding the bed the Tool can be used as a lever to operate the turnbuckles. 13. Store the Tool in the Velcro holder at the head of bed. 14. Make sure that the black woven safety strap located at the head end of the bed is securing the head gatch patient support surface to the base. CAUTION: Read #14 again. Unexpected movement of the patient support surface could occur if the safety strap is not in place resulting in possible personal injury. 15. Conf rm that the locks are set properly on the transport casters. 16. Carefully raise head end and lock to transport pole and secure in place with the transport latch bracket. Take care to avoid damage of the pendant. 17. Carefully raise foot end and lock to transport pole with the transport latch bracket. 18. Secure the AC power cord to holder. 19. Store the head and foot boards on the bed. 12

Operational Instruction Trendelenberg Function To access the Trendelenberg position press and hold the third Up or Down buttons on the hand pendant to tilt or reverse the bed as needed. (see f gure 10) Bed Elevation (Hi-Lo) Press and hold the fourth Up or Down buttons on the hand pendant to raise or lower the bed as desired. Foot (Up-Down) Press and hold the second Up or Down buttons on the hand pendant to raise or lower the foot as desired.

Head Elevation Press and hold the top Up or Down buttons on the hand pendant to raise or lower the head of the bed as desired. Maximum head elevation is 60 degrees (see f gure 11).
I.V. Poles If needed, I.V. support poles can be attached to each corner of the bed base frame.

Side Rail Width and Height Adjustments To attach the side rails to the bed, you must insert the side rail support frame into the horizontal tubes under the bed. Retract the stop pin by hand until you can pass the frame at least 2” into the bed gatch. Release the stop pin and continue to insert the support frame to the desired width. To adjust the width, retract the stop pin and slide the support frame to the in or out position. To remove the siderail support frame, slowly pull the frame out until it fully engages the stop pin. Manually retract the stop pin and continue to remove the arm. Normally, the siderail support frame should not be completely removed for anything other than setup or maintenance. WARNING: Be sure to fully reengage the stop pin when adjusting the width of the bed. Failure to do so could result in serious injury and or equipment damage. The Bariatric bed has a white plug at one end of each pullout. The location of the plug corresponds with the location of the pull pin that releases the pullout. With this feature the pull pins can be located more easily when the pullouts are extended to their widest position.

To lower the head side rail, hold the side rail at the handle and pull the spring loaded pull pin. Using the handle on the top of the side rail rotate the side rail toward the foot of the bed. Allow the side rail to rotate downward until it comes to a stop (see f gure 12).

To raise the head side rail, lift upwards on the handle at the top of the side rail. Allow the side rail to rotate completely upward until the spring loaded pull pin fully engages the stop (see f gure 13). WARNING: Be sure to fully reengage the stop pin when raising the head side rail. Failure to do so could result in serious injury and or equipment damage. To lower the foot side rail, hold the top of the side rail and pull the spring loaded pull pin. Rotate the side rail towards the foot of the bed. Allow the side rail to rotate downward until it comes to a stop (see f gure 12). To raise the foot side rail lift upward on the side rail. Allow the side to completely rotate upward until the spring loaded pin engages the stop. (see f gure 13) Bari Bed tWith Fold Down Side Rails To lower the fold down side rail, hold the top of the side and pull up the spring loaded pull pin. Rotate the side rail out of the way from the bed. Allow the side rail to rotate downward until it comes to a stop. To raise this type of side rail, lift upward on the side rail. Allow the siderail to rotate fully upward until the spring loaded pull pin engages the stop. WARNING: Be sure to fully reengage the stop pin when raising the foot side rail. Failure to do so could result in serious injury or equipment damage. Also, to avoid damage to side rail observe caster position. To raise the foot side rail, lift upwards on the side rail. Allow the side rail to rotate completely upward until the spring loaded pull pin fully engages the stop (see f gure 13). CPR Procedures Warning: This procedure will lower the head gatch in about 3-5 seconds. The emergency CPR release can be used to lower the patient faster than running the actuator. For maximum speed you should use the head down on the pendent control and pull the CPR release handle on the head gatch at the same time (see f gure 14). Warning: Consult a doctor before laying the patient f at or the patient’s lungs may collapse.
Figure 14 Patient Restraints WARNING: If patient restraints are used, they must be attached to some point on the bed that articulates when the gatches are raised and lowered. Failure to do so could result in the restraints injuring the patient. PATIENT TRANSFERS (from or to the bed) 1) Set the brake on all casters with spring locking lever pointing towards the foot end of the bed. 2) Adjust the height of the bed to match transfer height (It is best to always transfer down). 3) Lower the side rails or remove them. 4) Use a hospital sheet and appropriate number of personnel to lift the patient on/off the bed.

TRANSPORTING THE BED & PATIENT To transport a patient on the Bariatric bed: 1.) Adjust head elevation to the desired position for patient comfort. WARNING: Maximum patient weight in bed for transport is 1,100 lbs. (500 kg.). Transporting a patient above 1,100 lbs. can damage the bed and injure persons. 2) To ensure patient safety, the height of the bed should be lowered to its lowest position. WARNING: The bed high-low must be in the lowest position to prevent damage to the bed and provide for maximum stability. 3) Ensure siderails are locked in an upright position. WARNING: Side rails must be in the highest position to prevent patient from falling out of bed.. 4) Use an IV pole to mount solutions 5) Unplug the bed AC power cable, wrap the AC power cable around cable holder beneath the head board. Automatic battery power will temporarily allow the use of all bed functions until AC power is restored. 6) Unlock the two head end caster locks and one foot end caster lock. This will leave you with one directional lock caster engaged to assist in transport down a hall. 7) Use a minimum of two persons to transport the patient. CAUTION: Check the bed width and side rail adjustment for doorway clearance to prevent damage to the bed and door frames. CAUTION: Keep hands and feet clear of wheels during bed movement. Move the bed only at a slow walk. WARNING: Moving bed down inclines or excursion off a ledge (such as moving into an elevator or across a threshold) can result in damage to the bed and injury to persons NOTE: Remember to plug the bed into the wall outlet once transport of the bed is complete.

Cleaning, Storage, Service
CLEANING Clean the mattress provided with the bed by wiping down with a cloth or sponge using mild soap and water only. The hand pendant, scale display (optional), siderails, and bed frame can also be cleaned with a damp cloth or sponge and warm, soapy water. Do not use any scouring or abrasive cleaners on the bed. CAUTION: Do not hose down, high pressure spray, or place the bed in a shower as this may cause damage to the electronics system and void the warranty. If any bed electrical components should become wet, disconnect bed from the wall outlet until the device is thoroughly dry. STORAGE OF THE BED This equipment is provided with a battery back up. To insure the longevity of the battery back up we recommend that the bed be plugged into an appropriate wall outlet to keep battery charged when not in use for extended periods of time. Failure to do so could render the battery backup system inoperable. CAUTION: Store in a clean dry area. Dampness may affect electronic components and cause rust. SERVICE OF THE BED WARNING: If the bed looses any function or is damaged in any way stop using the bed immediately and refer it to a qualif ed service personel. Further equipment damage or personal injury may result. Refer to the service chart provided in this manual for basic problem solutions. Major service of this equipment should be referred to qualif ed personnel. Refer to circuit layout diagram and part identif cation diagram provided in this manual. The manufacturer of this equipment will provide assistance to the user’s appropriately qualif ed technical personnel to repair those parts of the equipment which are designated by the manufacturer as repairable. Use only replacement components which are approved and designated as acceptable by the manufacturer. INSPECTION The recommended frequency of inspection for the bed is once per month or before each new patient, whichever occurs f rst. FUSES The control box located under the left side of the head gatch has two line fuses located behind a round removable plug located on the underside of the box. A pair of pliers can grasp the “handle” of the plug and extract it from the box. The fuses are designated as follows: 5x20mm 1.6L250V Time-Lag fuse (1.6A/250V) Wickmann Series 19195 or equivalent

Troubleshooting Problem Check Bed does not operate
the following: > check all cords for fraying, cuts or damage and replace immediately. > check that the bed is plugged in, and that the wall outlet is working. > check for green, power on light, on top of control box, this indicates AC power to the box. If no light, and wall outlet is working, then fuses may be bad (requires tools). > if all or some of the relays in control box “click” when pendant is activated, and bed does not move, replace control box. > if no relay “click” can be heard from control box, check that pendant and other cords are completely plugged into control box. > if control box is found to be bad, check that no cords have been pinched or severed. Also check that actuators and all moving parts are not obstructed. Also check for damaged or dented actuators.
Bed only operates on battery > check that the bed is plugged in, and that the wall outlet is working. Only a portion of the bed > check all cords for fraying, cuts or damage and replace immediately. operates, while others don’t work > check that you are applying enough pressure to the pendant button to perform the function selected. > check that all plugs are securely installed into the control box and that the control box plug clamp is in place. > check that travel on all actuators and other moving parts are not obstructed. > try different location on control box. (ie: if head gatch does not work but knee gatch does, plug head gatch into knee gatch outlet. If head runs, then problem is in control or pendant, if no function, bad actuator. Scale does not operate (OPTIONAL)
> if no display, check batteries for possible replacement need. > check that the scale display is plugged into the load cells, and that the connector to each load cell (at each caster) is properly connected. > check that casters are properly aligned with bed and that caster brakes are set. > check that the cabling on any of the interconnects is not cut or crimped causing a signal problem. > check that there is clearance between the top of the caster and the bottom of the load cell housing.There must be some space between the top of the caster bearing cover and the housing.
If none of these suggestions results in full function returning to the bed then please, refer to properly trained service personnel for technical assistance. WARNING: If the bed looses any function or is damaged in any way stop using the bed immediately and refer it to a qualif ed service personel. Further equipment damage or personal injury may result.

Electromagnetic Compatibility (EMC)
This product has been tested to conf rm compliance with the appropriate EMC standards according to CISPR 11, Class B. Most electronic equipment is inf uenced by radio frequency interference (RFI), Caution should be exercised with regard to the use of portable communications equipment in the area around such equipment. While Burke has made every effort to ensure that RFI does not cause problems, very strong signals could cause a movement of a section of the bed. Such movement would be of very short duration and travel. It is the responsibility of the health care provider to conf rm that cell phones and other portable communications devices will not cause erratic behavior of other equipment. In the event that RFI does cause erratic behavior turn off the communications devices as soon as possible. For maximum safety do not operate the affected equipment until the source of the interference is removed. Report any malfunctions to qualif ed service personnel.
