Welcome Welcome to the Deltec Cozmo Insulin Pump Quick Reference Guide for Caregivers. This Quick Guide is provided to you because someone in your care wears a Deltec Cozmo insulin pump. The Deltec Cozmo insulin pump gives the wearer a simpler way to manage his or her diabetes. ®

This Quick Reference Guide provides abbreviated information on diabetes and instructions for a few common features of the Deltec Cozmo insulin pump. Please read the User Manual supplied with the pump, and the Instructions for Use supplied with the cartridge for detailed instructions, warnings, and cautions. Additional help is available by calling Smiths Medical Customer Support at 800-826-9703. ®

Your Role In Insulin Pump Therapy Communication is essential for a caregiver and should include: > A record of target blood glucose ranges, for before meals and after meals > Schedule and supplies for checking blood glucose > Emergency back-up plan, supplies for injecting insulin, and insulin pump supplies > Meal plan information including carbohydrate amounts for meals and snacks > Carbohydrate sources for treating hypoglycemia (low blood glucose) such as glucose tablets or gel > Guidelines for treating hyperglycemia (high blood glucose) > Signed prescriptions/instructions for insulin doses and glucagon > A plan for communicating with the individual’s healthcare provider or team (phone/pager numbers) and emergency contact numbers (home, work, and cell phone numbers) > A copy of the pump user’s Medical 504 Plan or Individualized Education Plan (IEP), if applicable > Deltec Cozmo insulin pump customer care number 800-826-9703 ®

What is Diabetes? Diabetes is a chronic disease in which the body does not make or properly use insulin, a hormone needed to convert food into energy. When the body’s naturally occurring insulin cannot properly metabolize blood glucose (blood sugar); the body loses its main source of fuel. There are two varieties of diabetes: > Type 1 - (Formerly called juvenile diabetes) The body does not produce insulin > Type 2 - Either the body does not produce enough insulin or the body’s cells ignore the insulin When a body doesn’t produce or use insulin properly, blood glucose levels–which are affected by sugars and carbohydrates in food–can get out of control.

Controlling Blood Glucose The goal of treating diabetes is to keep blood glucose levels within a target range that is determined for each person by their healthcare team. Blood glucose control promotes normal growth and development and helps delay or prevent complications. To keep blood glucose levels within target, people with diabetes should: > Check blood glucose levels at least four times a day > Be aware of and count carbohydrates in foods (sugars and starches) > Match insulin doses to carbohydrate intake, exercise or activity, and current blood glucose level

Deltec Cozmo Pump Basics ®

How Does an Insulin Pump Work? The Deltec Cozmo insulin pump is a small computerized device that contains a cartridge to be filled with insulin. The pump connects an infusion set, which is a thin, flexible piece of tubing with a tiny needle or cannula at the end that is inserted under the skin. The tubing carries the insulin from the pump’s cartridge through the cannula into the body. ®

The insulin pump is programmed to deliver small doses of insulin every few minutes 24 hours a day. This is referred to as basal insulin. The pump wearer can also deliver a larger dose of insulin (called a bolus) to match the carbohydrate content of food (a meal bolus) or to correct the level of insulin in the bloodstream (a correction bolus).

Loading a New Insulin Cartridge The Deltec Cozmo pump has a sensor that displays how much insulin remains in a cartridge and shows the amount in the upper left part of any home screen. The cartridge needs to be replaced when it is almost empty. ®

1 Remove the cartridge cap by turning it counterclockwise (left) 1/4 turn. If needed, turn the cartridge 1/4 turn counterclockwise (left) to unlock it from the push rod. Gently lift the cartridge from the chamber and dispose of it. 2 Fill a new cartridge with the desired amount of insulin (between 20 and 300 units) and attach the protective white cap. See the Deltec Cozmo Insulin Cartridge Instructions for Use supplied with the cartridge.

3 Insert the cartridge into the cartridge chamber with the Luer side (white cap) up. Turn the cartridge 1/4 turn clockwise (right) to secure it. (Do not force the cartridge into the chamber.)

4 On the Deltec Cozmo pump, start at any home screen (for this example, we are starting from Home Screen 1). Press Menu. ®

5 From this menu, use the Down arrow key to highlight Load, and press Select. This will bring up a Cartridge menu.

Be sure the infusion set is disconnected from the person’s body. Dispose of the used infusion set.

6 From the Cartridge menu, highlight Load, and press Select. If a warning that states “pushrod must move forward” appears, follow the instructions and press Select again.

7 The pump will then load the cartridge by pulling the cartridge into the chamber. Look through the cartridge viewing window to verify the cartridge is in place. 8 Reattach the cartridge cap on the pump by turning it clockwise (right) until the notch on the cap lines up with the raised line on the front of the pump. 9 Remove the white cap from the cartridge tip. Attach the infusion set Luer connector to the tip of the cartridge and twist it clockwise to tighten.

10 From the cartridge menu on the pump, highlight Fill Tubing, and press Select. Press Start. Filling is complete when insulin drips from the end of the tubing. (User must be disconnected from the infusion site.) Press Stop.

11 Have the person connect the infusion set to his or her infusion site. (Follow manufacturer’s instructions for use.) 12 From the cartridge menu, highlight Fill cannula, and press Select. Choose the amount needed then press Fill.

From the cartridge menu, highlight Reminder and press Select if you want to set the site change reminder. Use the arrow and Next keys to set the reminder day and time, then press Next. When complete, press Done on the Cartridge menu screen. The pump will return to the starting home page.

Delivering Boluses The Deltec Cozmo insulin pump can be programmed to deliver insulin boluses in several ways. Boluses can be programmed to deliver by either units of insulin or grams of carbohydrate. There are several types of boluses; you will most likely encounter meal boluses and correction boluses. ®

Giving Meal Boluses - By Carbohydrates At each meal or snack, you or the pump wearer enters the number of carbohydrates in the food to be eaten. The Deltec Cozmo pump calculates and recommends how much insulin is needed to cover those carbs. His or her specific insulin-to-carbohydrate ratio is pre-programmed into the pump and is used to calculate the bolus.

The CozFoods screen will appear. The Enhanced Meal Maker feature can only be used when programming boluses by grams of carbohydrates and must have been previously activated. ®

For each item in the meal or snack, select a food category. Use the Down arrow to highlight the appropriate category, then press Select. Each food category lists a number of food items. Use the Up or Down arrows to move through the list to the appropriate item and press Select. You will see the grams of carbohydrate in a serving listed. If needed, you can change the serving size by using the Up or Down arrows. When the food item is correctly listed, press Next. Repeat this process for every item in the meal or snack until you have entered the number of servings of every food item in the meal or snack. If you need to delete or change the serving size of an item, use the arrow keys to highlight it, press Next, and then change the serving size using the arrow keys.

The Carbohydrates screen will display all items selected and list the total grams of carbohydrate in the entire meal.

Test the person’s blood glucose level. Enter the current blood glucose using the Up or Down arrow keys. When you are finished, press Next. 4 The suggested bolus will display in units of insulin. When a person records a glucose reading that is or may drop below target, the Hypo Manager feature automatically recommends the number of fast-acting carbohydrates the individual should consume to bring his or her blood glucose back to target. By advising the exact number of grams of carbohydrate to eat, the Hypo Manager feature helps avoid overcorrection of hypoglycemic events. ™

If a Hypo Manager alert appears (see page 15), encourage the individual to follow the recommendation and consume the appropriate number of carbs or otherwise treat the low blood glucose as instructed by his or her healthcare provider. Re-check blood glucose levels after 15 minutes. ™

Selecting a Temporary Basal Rate The basal rate is the normal small dose of insulin the pump gives every few minutes 24 hours a day. Sometimes the pump wearer needs a different level of insulin and creates a temporary basal rate for a select period of time. For example, a person needs less insulin when physically active, but may need more when ill. He or she has likely already programmed in several temporary basal rates into his or her Deltec Cozmo insulin. To choose an existing temporary basal rate to accommodate the person’s activity level:

Replacing the Battery Batteries in a Deltec Cozmo pump need occasional changing, just as any device that runs on batteries does. The approximate amount of battery life is displayed on every home screen and an alarm goes off (see page 15) when battery power is low. Always keep extra standard AAA alkaline batteries on hand. To insert a new battery: ®

1 Loosen the battery cap by turning it counterclockwise (left) to open. Use a nickel in the slot on the battery cap to help loosen it. Remove the battery cap.

Remove the old battery and dispose of it. 2 Insert one standard AAA alkaline battery into the battery compartment, positive end (+ side) in first. If you insert it incorrectly, the pump will not start. 3 Place the battery cap back on and push down and turn the cap clockwise (right) to close it tightly. Again, use a nickel to tighten the cap. • You may hear a “chirping” sound if the battery cap is not tight.

• If the battery cap is not tight, the pump may power-down and power-up. Once the battery is inserted correctly, the pump turns on automatically, performs “self tests” to make sure all the systems are working properly, and then a home screen appears. If you insert a new battery and the pump doesn’t turn on, check to make sure that the battery is inserted correctly (the + side goes in first). If it still doesn’t turn on, try a new battery.

Disconnect Feature The Disconnect feature lets the wearer remove the pump for 15 minutes up to 2 hours. The Deltec Cozmo pump suggests insulin boluses when the pump wearer detaches from and reconnects to the Deltec Cozmo insulin pump to replace projected missing basal insulin.

1 To tell the Deltec Cozmo insulin pump that you want to disconnect, start at any home screen (for this example, we are starting from Home Screen 1). Press Menu.

Press Next. 4 The pump will indicate how much basal insulin will be missed while the pump wearer is disconnected. Indicate up to 50% of the projected missed basal insulin (by pressing the Up or Down arrows) to deliver as a bolus before disconnecting. Press Next. 5 The pump will remind the pump user to test his or her blood glucose. Press the Up or Down arrow to select Yes, then press Next. Enter the result and press Next. Press Deliver to give the projected missed basal bolus. Once delivered, the person in your care can remove the pump and infusion set. 6 You’ll notice that the DIS home screen appears and indicates the amount of time left until the planned re-connection. 6a An alarm will alert you at the designated re-connection time. (A person can always reconnect earlier.) Press OK. 7 The individual should reconnect the infusion set and Deltec Cozmo insulin pump. Once connected, press Yes. ®

8 The Deltec Cozmo insulin pump will show the remaining amount of basal insulin missed while disconnected. You can choose to give any percentage of the basal insulin not yet delivered as a reconnect bolus at re-connection. Press the Up or Down arrows to indicate the appropriate percentage to give as a bolus. Press Next. ®

You’ll be asked if you want to enter a new blood glucose value. Press the Up or Down arrow to highlight Yes, then press Next. Press the Up or Down arrow to enter the result and press Next and follow the directions.

To deliver the remaining missed basal insulin, press Deliver.

Alerts and Alarms The Deltec Cozmo insulin pump has a number of optional alerts and alarms that help the pump wearer keep his or her insulin treatment on track. You may encounter any of the following alarms: ®

Blood Glucose - (1) This alert reminds Deltec Cozmo pump wearers to test their blood glucose. If the person in your care notices a Blood Glucose alert, have him or her perform a blood glucose test. ®®

Missed Meal Bolus - (2) This alarm can be pre-programmed to alert the Deltec Cozmo pump wearer when no meal bolus is taken within a specific time period. If the person in your care notices a Missed Meal Bolus alert, he or she may need to take a meal bolus to cover a meal already eaten.

Site Reminder - (5) This alert can be set to remind people to change their infusion sets every few days. If the person in your care notices a Site Reminder alert, it is time to change his or her infusion set and site. (Follow manufacturer’s instructions for use when inserting a new infusion set.)

Every home screen shows the amount of insulin you have left in your cartridge (Fig. A), the approximate power left in your battery (Fig. B), the name of your pump (Fig. C), and an icon shaped like a little house that tells you which home screen is in use (Fig. D). In the Model 1800 Deltec Cozmo insulin pump, there are three permanent and six optional home screens. ®

• Home screen 1 shows the current basal pattern that is active (Fig 1A), and the rate at which the pump is delivering insulin (Fig 1B). • Home screen 2 displays information on your last bolus including the amount and the time it was completed (Fig 2A). From this home screen, you can also review all bolus history information and a Therapy Effectiveness Scorecard by pressing Stop-Rev (Fig 2B). • Home screen 3 displays the date and time (Fig 3) and personal information that has been entered using the CoZmanager software, such as the name of the pump, emergency information, phone numbers, or other useful information. ®

Additionally, there are six optional home screens. The following screens may or may not appear on the person’s pump. • The Insulin on Board, or IOB, home screen (Fig 4) shows the amount of insulin “on board,” that is, the insulin remaining active in the person’s body, since the last bolus (Fig 4A). It also shows the time remaining in the duration of insulin action (Fig 4B). This screen is updated every minute.

• The Blood Glucose, or BG, home screen (Fig 5) displays the time and date and the results of the most recent blood glucose test. • The Site Reminder, or SR, home screen displays the time and date you’ve set for your next infusion site change (Fig 6). If this date isn’t updated at each site change, it will show an expired notice. • The Temporary Rate, or TR, home screen displays the name (Fig 7A) and the remaining duration (Fig 7B) of any temporary rate that is active. • If the Weekly Schedule feature is in use, you’ll see the day of the week inside the house icon (Fig 8A), the current basal pattern (Fig 8B), and the alert set in use (Fig 8C), if you are using one. • The Disconnect, or DIS, home screen appears if you are using the Disconnect feature, indicating that the pump is currently disconnected from your body (Fig 9A). It also shows the time remaining until the person will need to reconnect (Fig 9B) to the Deltec Cozmo pump. ®

To move from one home screen to the next, press the up or down arrow from any home screen to move to the next.
